security requirements and background check form Wage Determination Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F2R0S14111A001 is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. CLIN 0001 - 36 Each - IAW attached Statement of Work (SOW), provide a service contract for qualified individual or firm to conduct the Motorcycle Safety Foundation (MSF) "Basic Rider Course" for military and DoD civilian motorcycle operators assigned to Laughlin AFB. Training must be conducted in accordance with all current MSF and USAF directives pertaining to motorcycle operators. The instructor(s) must be certified by MSF to teach the course. Period of performance will be base year 1 Oct 14 - 30 Sep 15. There are four (4) option years for this CLIN. CLIN 0002: - 30 Each - IAW attached SOW, provide a service contract for qualified individual or firm to conduct the Motorcycle Safety Foundation (MSF) "Advanced Rider Course" for military and DoD civilian motorcycle operators assigned to Laughlin AFB. Training must be conducted in accordance with all MSF and Air Force directives pertaining to motorcycle operators. The instructor(s) must be certified by MSF to teach the course. Period of performance will be Base Year: 1 Oct 14 - 30 Sept 15. There are four (4) option years for this CLIN. Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. This acquisition is 100% Small Business Set-Aside and is under North American Industry Classification System code 611692 with a small business size standard of $7.5M dollars. This purchase request is subject to availability of funds and incorporates FAR Clause 52.232-18, Availability of Funds (April 1984). Please identify your business size in your response based upon this standard. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76 effective 25 Aug 14, the Defense Federal Acquisition (DFARS) Change Notice 20140828 effective 28 Aug 14, and Air Force Acquisition Circular (AFAC) 2014-0421, effective 21 Apr 14. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. The provision FAR 52.212-1 Instructions to Offerors - Commercial Items (APR 2014) and the applicable addendum apply to this acquisition. The addendum references FAR 52.212-1(b)(8) and is hereby tailored as follows: FAR 52.212-1 Addendum SUBMISSION OF COMPLETE QUOTES: All quotes must be complete, self-sufficient, and respond directly to the requirements of this solicitation. Failure to furnish a complete quote, at the time of quote submission, may result in the quote being unacceptable to the Government and eliminated from consideration for award. A complete quote must include the following: Motorcycle Safety Foundation certification; 52.212-3 Alt I, Offeror Representations and Certifications - Commercial Items (MAY 2014); 52.219-28 Post-Award Small Business Program Representation (JUL 2013); 52.222-22 Previous Contracts and Compliance Reports (FEB 1999); 52.222-25 Affirmative Action Compliance (APR 1984); 252.209-7999(Dev) Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law (JAN 2012). Offerors shall include with their offer a completed copy of the provision FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (May 2014) and Alternate I (May 2014). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Award will be made to the lowest offer in compliance with attached Statement of Work (Attachment 1), and can be determined to be responsible. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Your purchase order/contract number or invoice will be required to inquire status of your payment Contractors are advised that the Commander has placed restrictions on the smoking of tobacco products in AETC facilities. AFI 10-42, Tobacco Use in the Air Force, and its AETC Supplement 1, outline the procedures used by the Commander to control smoking in our facilities. Contractor employees are subject to the same restrictions as government personnel. Smoking is permitted only in designated smoking areas. The contract will include four 1-year option periods and FAR Clause 52.217-8, Option to Extend Services, with the potential to extend the contract an additional 6 months beyond the awarded contract performance period. In the event the Government requires continued performance, the Government's evaluation of prices proposed will be considered fair and reasonable as applied to an extension of services. Should an extension of services be required rates will be adjusted only as result of revisions to applicable prevailing Department of Labor wage rates. This purchase will incorporate one or more clauses by reference, with the same force and effect as if they were given in full text. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management (SAM) www.sam.gov.
