Description

The Stratton Albany VA Medical Center, within the VA Health Care Network Upstate New York (VISN 2), has a need for Community Based Outpatient Clinic (CBOC) services providing primary care services in a private hospital, office, or clinic environment for the Veterans of Franklin County, New York. Contractor is required to provide a clinic located within the city limits of Malone, NY. Request for Proposal (RFP) VA528-14-R-0481 will be available for download from this website on or about October 1, 2014. No hardcopies will be sent out. Questions must be submitted in writing via email to the Contract Specialist at Allen.Johnson2@va.gov. Questions must be received a minimum of ten (10) business days before the closing date listed in the RFP. If warranted, responses to inquiries will be incorporated into a written amendment posted to the Federal Business Opportunities website at http://www.fbo.gov. Verbal questions will not be addressed. Offerors are encouraged and are responsible for routinely checking this website for any solicitation amendments that may be issued throughout the proposal period. This requirement is being solicited as unrestricted. The NAICS Code for this requirement is 621498 - All Other Outpatient Care Centers and the Size Standard is $19 Million. Contractors are required to be registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM prior to contract award.
