ADDENDA #1 to RFQ VA255-14-Q-1524 dated October 4, 2014 The attached document (ADDENDA 1 to RFQ VA255-14-Q-1524) provides questions or clarifications received from interested vendors concerning the solicitation and answers provided in response. The information contained in the attachment is to be considered as addenda to the solicitation and is to be considered in making a response. ORGINAL RFQ BIOMEDICAL ENGINEERING CONSULTING SERVICES for the St Louis MO VA Medical Center Request for Quotation (RFQ) - in accordance with FAR part 12 (Acquisition of Commercial Items) and FAR Subpart 13.5 (Test Program for Certain Commercial Items), Network Contracting Office (NCO) 15 is soliciting quotations for furnishing BIOMEDICAL ENGINEERING CONSULTING SERVICES for the St Louis MO VA Medical Center. Services will include project management, investigation of reported safety issues or incidents with medical devices within VA Health Care Systems, report writing and presentation. Per the attached RFQ document #VA255-14-Q-1524 Statement of Work (SOW), terms and conditions. -The procurement will be a 100% Veteran Owned Small Business (VOSB) Set-Aside. -The NAICS code for this requirement will be 541611 with a size standard of $14 million. -Quotations will be evaluated on Technical Excellence, Past Performance and Price in accordance with the evaluation criteria identified in the solicitation. -Award will be Best Value. -The Government anticipates awarding a one (1) year contract with four (4) one (1) year option periods. - As specified in section E.1 of the RFQ, please provide the following with each response: (1). Offer Form (Standard Form 1449). Fill in blocks 12, 17a, 30b and 30c, sign Block 30a. Return a completed copy of the form with your response. (2). Section B.1. Contract Administration Data. Fill in Section B.1 (a) (1) contractor information; section (k) (1) unpaid tax certification; section (k) (2) responsibility certification; section (k) (3) representations and certifications; and section (l) acknowledgement of any solicitation amendments. Return a completed copy of the section with your response. (3). Representations and Certifications. If you do NOT have a current Representations and Certifications on-line at https://www.sam.gov a copy of FAR 52.212-3 (Offeror Representations and Certifications - Commercial Items (MAY 2014), found in Section E.14 of the solicitation, must be completed and returned with your response. (4). Price. Please complete and return Section B.2 Price Schedule. Complete each Contract Line Item Number (CLIN) for the Base Contract Period and for each of the four (4) Option Years. Attach a copy of the completed section to your response. (5) Technical Excellence. (A). SUBFACTOR A - Please provide supporting documentation showing adequate resources, staffing, offices/buildings, capability, experience, responsibility and integrity to meet the technical capability requirements of the solicitation and the resulting contract. (B). SUBFACTOR B - Please provide a resume for each individual you are proposing for furnishing Project Management, Biomedical Engineering and Subject Matter Expert services as required by section B.4. - Statement of Work (SOW) (d) SERVICES TO BE PROVIDED (e) CONTRACTOR EXPERIENCE REQUIREMENTS. (6) Past Performance. (A). Please provide a list of no more than three (3), of the most relevant contracts performed for Federal agencies and commercial customers for furnishing services similar to those specified in this Request for Quotation (RFQ) within the last 3 years. Please provide the following information: (i). Company name/Point of Contact with Phone Number. (ii). Service provided. (iii). Contracting Agency/Customer. (iv). Contract Dollar Value. (v).
