- STATEMENT OF WORK Grounds Maintenance Service Alaska VA Healthcare System Borough of Anchorage, Alaska 1. Contract Title. Grounds Keeping Services at Domiciliary & Transitional Housing Units 2. Background. This is a Firm Fixed Priced contract for a Base year plus four (4) Option Years to provide grounds maintenance services. This is a contract to provide grounds keeping services for the VA Domiciliary at 3001 C Street, and the VA Transitional Housing Units at 709 N. Hoyt Street, 932 & 944 East 12th Avenue, City of Anchorage in the Borough (County) of Anchorage, Alaska. 3. Scope. Contractor shall furnish all management, labor, and transportation required to perform grounds keeping services and selected snow removal for the Domiciliary Homeless Veterans Service and the Transitional Housing Residences in accordance with the terms and conditions of this contract. These services shall include but not be limited to mowing, edging, soil management, weeding, planting/transplanting of vegetation pruning, fertilizing, cleaning sidewalks and removing snow/ice from sidewalks and entryways. Also included are painting for picnic tables, outside benches, redwood landscaping timbers around the flower beds, and managing sheds as needed. 4. Specific Tasks. A.Maintenance: 1.Maintenance and upkeep of all VA owned tools and equipment (power, manual, and hand operated.) a.On an as needed basis changing of oil, filter, spark plugs, and sharpening of blades will be completed to maintain power equipment in good working condition. Clean all tools and equipment used during the work day before storing them in their appropriate places at the end of the work day. All snow equipment shall be maintained in the same manner. 2.Upkeep of outside furniture (repair, staining, and painting..) a.Outdoor furniture shall be kept in good operating condition. Preventative maintenance on all outdoor furniture shall be completed throughout the operational season. Starting and/or painting of the furniture shall be done once a year during the summer months in accordance with paint manufacturer's specifications. 3.Upkeep of all sheds (repair, staining, and painting.) a.Utility sheds are to be kept in good condition. Preventative maintenance shall be ongoing throughout the year. Staining and/or painting of the furniture shall be done once a year during the summer months in accordance with paint manufacturer's specifications. B.Lawn, Sidewalk, Driveway, and Flower Bed Care: 1.Mowing of all lawns. a.Lawns are to be mowed at a minimum of once a week with the results being an even and uniform appearance of between 2 - 4 inches. Clippings shall be disposed of in the back parking lot dumpster near the out buildings designated by the COTR. 2.Edging of all sidewalks and lawn area. a.Edging is to be done at a minimum of once a week with the results being a clean cut edge in relation to the sidewalk, driveways, or curb. Clipping shall be disposed of in the back parking lot dumpster near the out buildings. 3.Weeding of all flower beds. a.All flower beds around buildings, parking lots, and fencing are to be weed free during the growing season. 4.Planting of annual flowers and/or bushes. a.The VA Domiciliary will provide all flowers or bushes to be planted. To ensure the health of all new flowers and bushes the contractor shall complete planting in a timely manner. 5.Transplanting of plants and flowers. a.Transplanting shall be performed so as to ensure the healthiness of the transplanted plants and/or flowers. 6.Pruning of trees and/or bushes. a.Pruning shall be performed weekly to ensure correct form and growth of the trees and bushes. 7.Watering all flowers, trees, bushes, and lawn areas. a.Watering shall be performed on a daily basis throughout the week in such a way as to ensure total coverage of areas where plant growth is present and encouraged. 8.Thatching of lawns. a.Thatching shall be done during the spring to promote healthy growth of the lawn. Clippings shall be disposed of in the back parking lot dumpster near the out buildings as designated. 9.Fertilizing lawn areas. a.Fertilizing shall be done during the spring after the snow melt and fall prior to the first snow fall. 10.Upkeep of parking lots, sidewalks, curbs, and street areas. a.Parking lots, sidewalks, curbs, and street areas shall be kept clean of trash and plant debris. Curbs are to be painted in such a manner and color so as to ensure the appropriateness of its application. All parking lots are to be striped suitable in position and color for vehicle parking areas once a year. 11.Snow removal and de-icing of building entryways and sidewalks. a.For the purposes of this contract, a "snowfall event" shall commence after an accumulation of snow greater than 2 inches is observed at the premises, shall continue while continuous accumulation of snow is observed at the premises, and shall terminate after accumulation has ceased for 2 hours at the premises. The contractor shall monitor local weather conditions and perform one snow removal of all sidewalks and entryways after a snowfall event is determined to have commenced. If there is a further accumulation of 2 or more inches of snow during the snowfall event, the contractor shall return and perform one additional snowfall removal of same within 24 hours of the termination of the snowfall event. De-icing shall be applied as necessary no later than 7:30 AM 7 days a week, and shall be done throughout the day as necessary to ensure clear passage on sidewalks and entryways. Removal of snow and de-icing in parking lots is not part of this contract. 5. Performance Monitoring (if applicable). COR or COR representative at the Domiciliary will monitor timeliness and quality of work per the Quality Assurance Surveillance Plan (QASP), and report any issues to the Contractor for correction. If unsatisfactory performance continues, the COR shall fill out a Contract Discrepancy Report (CDR) (attachment 1), which shall be sent to the contractor. The contractor shall sign the CDR and provide a Quality Action Plan to fix the unsatisfactory service to the COR and the Contracting Officer. 6. Security Requirements. The contractor employees shall not have access to VA sensitive or computer information and will not require routine access to VA Facilities. The contractor employees shall require intermittent access only and will be escorted by VA employees while at VA Facilities. No background investigation is required 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Equipment (see attached equipment list): 1.The VA will provide all necessary lawn care tools such as, but not limited to, shovels, rakes, hoes, lawn mowers, edgers, lawn tractors, lawn tractor attachments for snow removal, and spreaders. Contractor is responsible for care and reporting eminent failure of equipment and tools to the COR. Fertilizer, plants, and seed: 1.The VA will provide all necessary fertilizers, plants, and seeds for the continued growth or transplantation of all vegetation included in this contract. Fuels, oils, and filters: 1.The VA will provide all necessary fuels, oils, and filters for the execution of this contract. 8. Other Pertinent Information or Special Considerations. None. 9. Risk Control All work shall be performed in accordance with OSHA guidelines with health and safety of contractors, staff, and veterans first and foremost in mind. Contractor shall discuss any anticipated safety issues with COR representative at the Domiciliary. 10. Place of Performance. Borough (County) of Anchorage, City of Anchorage; VA Domiciliary, 3001 C Street. Transitional Housing Residences at 709 N. Hoyt Street, 932 E. 12th Avenue, and 944 E. 12th Avenue. 11. Period of Performance. Base Year:01 October 2014 through 30 September 2015 1st Option Year:01 October 2015 through 30 September 2016 2nd Option Year:01 October 2016 through 30 September 2017 3rd Option Year:01 October 2017 through 30 September 2018 4th Option Year:01 October 2018 through 30 September 2019 12.
