Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number VA262-14-Q-1350 is issued as a Request for Quote (RFQ). The NAICS Code is and 621511 Medical Laboratory. The Government's intent is to make a multiple award, firm-fixed price contract to the responsive and responsible Lowest Price Technically Acceptable offeror. The contractor is responsible for providing all labor, supplies, equipment, shipping materials and supervision necessary to provide laboratory work related to Genetic and Genomic testing, as stated in the Statement of Work (SOW). It is contemplated that the Period of Performance will be one 12-Month Base Period and one (1), 1 - Year Option Period. The following factors shall be used to evaluate offers received: (1) Technical Expertise; 2) Management Capabilities 3) Past Performance. To be technically acceptable, the offeror must adhere to the "B.4 Instructions, Conditions and Other Statements to Bidders/Offerors" of the solicitation. Technical factors will be evaluated for acceptability in accordance with FAR 15.101-2, Lowest price technically acceptable source selection process. As part of price evaluation, the Government will evaluate its option to extend services (see FAR Clause 52.217-8) by adding six (6) months of the offeror's final option period price to the offeror's total price. Offerors are required only to price the base and option periods. The government may choose to exercise the Extension of Services at the end of any performance period (Base or option periods), utilizing the rates of that performance period. Request for Information (RFI) must be submitted in writing to the Contracting Officer, emailed to guiljohn.alcantara@va.gov, no later than August 03, 2014 at 12:00PM, PST, to allow replies to reach all prospective offerors in a timely manner. Offers are due no later August 09, 2014 by 5:00PM PST. Only offers submitted through electronically shall be considered. Email offers to: guiljohn.alcantara@va.gov.
