- This solicitation VA246-14-Q-0693 is issued as a request for quote (RFQ). Award will be made using simplified acquisition procedures in accordance with FAR Part 13. No telephone requests will be accepted. Only completed SF1449 Solicitations received directly from the requester are acceptable. It is the Offeror's responsibility to ensure that the quote is received. The anticipated award date is October 1, 2014. The associated North American Industry Classification System (NAICS) code for this procurement is 311811. This action is for full and open competition. If offeror has a FSS/GSA contract, please include it in your offer. The Department of Veterans Affairs, Beckley, WV, is seeking to place Dairy orders on Monday and Thursday for Tuesday and Friday delivery, for the period of October 1, 2014 through September 30, 2015, with a possibility of two options years. Milk, Whole, ½ Pint Milk, Skim, ½ Pint Milk, Lowfat, ½ Pint Orange Juice, Unsweetened, 4 oz. Orange Juice, ½ Gallon GP A, Buttermilk, ½ Gallon GP A, Cottage Cheese, 5 lb. container Lactaid #609, Quart Interested and capable respondents are requested to provide the information below. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number (www.dnb.com ), list of available products, the geographical location the respondent is able to service, and a list of the respondent's establishment's certifications from private, State and/or local agencies documenting the safety and sanitary conditions and procedures. Small Disadvantaged d. HUBZone e. Woman Owned f. Small Business Extremely Important: The VAMC Beckley, WV is located in a rural and mountainous region. Respondents must be able to make timely deliveries even in extreme weather conditions. Please notate the distance in miles from the VA that you are located or if you have a centrally located distribution center.
