- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-VA251-14-Q-0636 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76. No telephone requests will be accepted. Only written, faxed or email requests received directly from the requester are acceptable. If requesting by FAX, you may send your request to 734-845-3268, ATTN: Robert Kay, email, robert.kay@va.gov. The North American Industry Classification System (NAICS) number is 325120 and the business size standard is 1,000 employees. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. The proposed contract listed here is Set-Aside 100% for Small Business. The Contractor shall provide all necessary labor, materials and transportation as necessary to deliver One (1) 50 pound block dry ice, sliced into 1.5 inch layers delivered every Monday, Wednesday, Friday via refrigerated truck to the VA Ann Arbor Healthcare System for the period of October 1, 2014 through September 30, 2015, with four (4) additional one (1) year options. Delivery to VA Ann Arbor Healthcare System, Building 31/Room 105, Research Service, 2215 Fuller Road, Ann Arbor, MI 48105. Deliveries made during normal working hours 8:00AM to 3:30PM except for Federal Holidays. The VA has the option to Increase, Decrease or Cancel any delivery given one (1) day advance notice. Base Year $________________, Option Year #1 $ _____________, Option Year #2 $____________, Option Year #3 $ _____________, Option Year #4 $____________, AGGREGATE TOTAL PRICE $_______________. FOB destination. Invoicing shall be monthly. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: IL 001AL-11-15-A, Limitations on Subcontracting, IL 001AL-11-15-B, Subcontracting Commitments, 52.212-2, Evaluation-Commercial Item, the Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. To be technically acceptable the item shall meet the Government's specifications. Technical & Past Performance will be evaluated slightly higher than price. The following additional clauses apply: SAM Clause: Attention: You must be registered in the System for Award Management (SAM) to be considered for award. (C) Lack of registration in the database will make an offeror ineligible for award.
