- Request for Quote/Pricelist Statement of Work COMBINED SYNOPSIS/ SOLICITATION FOR COMMERCIAL ITEMS (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M3E34206AW01 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76, Dated 25 Jul 2014. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 811310 with an $7.5 million size standard. (v) Contractors shall submit a lump sum quote for three GYRO-TRAC machine repairs. The Pricelist A Format is attached to this combination solicitation/synopsis and will be used to submit quotes. After award, this document will become part of the contract. (vi) Repairs for three GYRO-TRAC machine located on Joint Base Charleston, Air Base, South Carolina. (See attached statement of work) (vii) Repairs must begin within 10 calendar days of award and be completed NLT 60 calendar days of award. (End of clause) • DFAR 252.209.7999, Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law reads as follows: REPRESENTATION BY CORPORATIONS REGARDING AN UNPAID DELINQUENT TAX LIABILITY OR A FELONY CONVICTION UNDER ANY FEDERAL LAW (DEVIATION 2012-O0004) (JAN 2012) (a) In accordance with sections 8124 and 8125 of Division A of the Consolidated Appropriations Act, 2012, (Pub. (End of provision) • AFFARS 5352.223-9001, Health and Safety on Government Installations (NOVEMBER 2012) (a) In performing work under this contract on a Government installation, the contractor shall: (1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract. • AFFARS 5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (AUGUST 2007) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or security police for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. FAR 22.1016 requires us to include the clause at FAR 22.222-42 "Statement of Equivalent Rates for Federal Hires" in our service contracts valued at $2,500 or more which are subject to the Service Contract Act. 2. The clause itself requires fill-ins showing the class of employee(s) and wages and fringe benefits to be paid. 3. To determine the federal equivalents: a. Go to the SCA Directory of Occupations at the internet link: http://www.dol.gov/whd/contracts/sca.htm. Then scroll down the page to the Guides menu. Select the link titled "SCA Director 5th Edition, Occupational Index". b. The occupations are listed alphabetically. Find the class or classes that will be needed to perform the service in your requirement. Make a note of the Occupational Code Number, title, grade and page beside the class(es) that you select. For Example: 11150 JANITOR WG-2 Page 30 c. Now back out of the table and on the same website, select the link titled "Service Contract Act Directory of Occupations, 5th Edition". Look up each page number for the occupations that you have just selected and read the description for the class. Be sure it fits your requirement. If it does not, then you must do more research to locate the occupation that best fits your requirement. d. Next look up the hourly wages on the appropriate federal employee wage schedule at the internet link: http://www.cpms.osd.mil/wage/wage_schedules.aspx. For (Blue Collar) WG rates: Select "Current APF Wage Schedules". Next select "South Carolina" and then open the "(119) Charleston, South Carolina" and then select "119R-21 Sep 2010.html" document for the latest Wage Grade rates table. There are three tables on this table. You will want to only refer to the WG-Rates table. Wages paid blue collar employees shall be the basic hourly rate for each class. Using the example above for a Janitor locate the WG-2 hourly rates for the Step 2 and Step 3. Use Step 2 for non-supervisory positions and Step 3 for supervisory positions in accordance with FAR 22.1016. Therefore, the Janitor WG-2 Rate for a Step 2(non-supervisory) is $12.07/hour and for the Step 3 the rate is $13.60/hour. For (White Collar) GS rates: Select "GS Pay Tables". Next select "General Schedule and Locality Pay Tables" and then open the "Hourly Rate" document for the latest General Schedule (Base) table. Wages paid white collar employees shall be an hourly rate for each class. Using the example above for a Janitor locate a GS-2 hourly rate, the rate shall be obtained by dividing the general pay schedule step one biweekly rate by 80 in accordance with FAR 22.1016. Therefore, the Janitor GS-2 Rate for a Step 2 is $9.82/hour and for the Step 3 the rate is $10.14/hour. 4. Depending on the latest DOL Annual Training Handout, use the fringe benefit percentage that was briefed. The current Fringe Benefit % rate is 32.45% of the hourly rate of pay. 5. You would fill in FAR 52.222-42 in the BLUE type as follows: EXAMPLE: 11150 JANITOR WG-2 $12.07 per hour + 32.45% est. for fringes
