Description
DRAFT PERFORMANCE WORK STATEMENT 1.BACK GROUND The Pathology & Laboratory Medicine Service (P&LMS) office at the George E. Wahlen VA Medical Center located at 500 Foothill Drive, Salt Lake City, UT 84148 requires Preventive Maintenance and Emergency Repair services on Tissue TEK equipment. 2.SCOPE OF WORK The Contractor shall provide all parts, materials, labor, supervision, test equipment, technical manuals, and transportation necessary to perform one preventive maintenance service visit per year and any emergency repair service as required by the Government for the following VA owned equipment: a.TISSUE TEK VIP6 / 6030 TISSUE PROCESSORS (S/N 60300857) b.TISSUE TEK VIP6 / 6030 TISSUE PROCESSORS (S/N 60300858) c.TISSUE TEK PRISMA / 6130 AUTOMATED SLIDE STAINER (S/N 61300872-1012) d.COVERSLIPPER - Linked FILM / 4740 (S/N 47400800) 3.CONFORMANCE STANDARDS All services provided under this contract must be performed in conformance with the Federal Drug Administration (FDA), Occupational Safety and Health Administration (OSHA), College of American Pathologist (CAP), and Original Equipment Manufacturer standards and specifications. 4.HOURS OF WORK a.Hours of work for preventive maintenance and repairs are defined as Monday through Friday from 8 a.m. to 5 p.m., excluding federal holidays. b.Contractor is not responsible to provide services on the following federally recognized holidays: New Year's DayLabor Day Martin Luther King's DayColumbus Day President's DayVeteran's Day Memorial DayThanksgiving Day Fourth of JulyChristmas Day Also, any other day declared by the President of the United States to be a national holiday. 4.PREVENTIVE MAINTENANCE a.The Contractor shall perform one annual preventive maintenance procedures during each year of the contract as arranged with the station Point of Contact (POC) or a designated Biomedical Engineering Department Biomedical Technician from the VA SLC Engineering Department (designee). Preventive maintenance procedures shall be in accordance with the published preventive maintenance manuals for the equipment listed in the schedule. The Contractor shall utilize the Original Equipment Manufacturer's established procedures and checklists, (or Contractor-supplied equivalent satisfactory to the POC or designee. a.The Contractor shall provide a Field Service Report to the POC or designee at the completion of each preventive maintenance procedure. The report shall include: "date and time notified "date and time of arrival "serial number, type, and model number of equipment "time spent for repair "description of malfunction and repair "proof of repair that includes documentation of a sample run of quality control verifying acceptable performance b.The Contractor shall provide a Preventive Maintenance Report to the POC or designee at the completion of each preventive maintenance procedure. The report shall include: "Cleaning of equipment "Completing Original Equipment Manufacturer field service updates for operational and reliability change notices "Calibrating and lubricating the equipment "Performing remedial maintenance of non-emergent nature "Testing and replacing faulty and worn parts "Inspecting/replacing tubing for wear and fraying "Inspecting all mechanical components including, but not limited to; cables and mounting hardware, chains, belts, bearings and tracks, interlocks, and motors for mechanical integrity, safety, and performance to Original Equipment Manufacturer specifications "Returning the equipment to operating condition defined in Original Equipment Manufacturer specifications "Replacing any Original Equipment Manufacturer labels, decals, or warning tags that are not legible "Providing documentation of services performed c.The Contractor shall notify the POC or designee of the existence or development of any defects in, or repairs to the equipment covered under this contract which the Contractor considers he/she is not responsible for under the terms of the contract (such as operator misuse). d.A set preventive maintenance schedule will be created by the POC and the Contractor after contract award and shall be adhered to. Any deviation from the set schedule will be agreed to by both the government and the contractor. e.The contractor shall not leave a service call open pending verification that repairs are completed by the contractor's maintenance personnel. The contractor shall treat each notification for an emergency/repair service call from the Government as a separate and new service call. f.The Contractor shall ensure that all components/parts utilized to perform preventative maintenance and/or repairs meet manufacturer's specifications, be noted on the Field Service Report and have a 90 day warranty. Refurbished replacement parts will not be acceptable. 5.EMERGENCY REPAIR SERVICE a.Emergency repairs shall be performed after initial telephone notification that the equipment is inoperative. The contractor shall provide the Government with a designated point of contact and telephone number and shall make arrangements to enable his maintenance representative to receive such notification. b. The Contractor shall provide a Field Service Report to the POC or designee at the completion of each repair service call. The report shall include: "date and time notified "date and time of arrival "serial number, type, and model number of equipment "time spent for repair "description of malfunction and repair "proof of repair that includes documentation of a sample run of quality control verifying acceptable performance c.The contractor shall be on-site at the George E. Wahlen VA Salt Lake City Health Care System Pathology & Laboratory Medicine Service to perform emergency repair service within 24 hours after receipt of initial telephone notification. The Contractor shall provide coverage for repairs Monday through Friday from 8 a.m. to 5 p.m., excluding federal holidays or as otherwise arranged with the Contracting Officer Representative. d.The Contractor shall provide technical support service to assist in troubleshooting 24 hours per day, 7 days per week including Federal holidays. g.The Contractor shall ensure that all components/parts utilized to perform preventative maintenance and/or repairs meet manufacturer's specifications, be noted on the Field Service Report and have a 90 day warranty. Refurbished replacement parts will not be acceptable. 6.PARTS a.The Contractor shall furnish all parts necessary to perform the preventative maintenance or emergent repair on the equipment, covered by this contract, in accordance with the Conformance Standards Section (Section 2). The Contractor shall have ready access to new standard parts (manufactured, supplied by the manufacturer, or equal thereto). All parts supplied shall have full compatibility with existing equipment. Documentation of intended parts source(s) shall be provided to the POC or designee upon request. NOTE: Any additional charges outside the scope of this contract to be claimed shall be approved by the Contracting Officer. b.The contractor shall have in-stock or have the ability to provide all necessary parts, components, assemblies, modules and equipment to restore any malfunctioning equipment to full performance within 24 hours of the originating call. Full performance is defined as all defective parts having been replaced with parts equivalent to or better than the original parts and meets or exceeds the manufacturer's original performance specifications. The contractor shall supply and bear the cost of all parts, lubricants, cleaning supplies and tools required for preventive maintenance and, as a result of normal wear and tear, restore equipment to full performance. NOTE: Parts, considered to be the manufacturer's recommended parts, are to be replaced in the course of a recommended preventive maintenance inspection. All parts shall meet the original manufacturer's design specification 7.CONTRACTOR TECHNICIANS a. The Contractor's Technician shall be a certified and experienced Field Service Engineer capable of making any required repairs or performing preventative maintenance on all equipment listed in this Performance Work Statement (PWS). The Technician shall have a minimum of one (1) year of experience providing preventive maintenance and emergency repair services on the same make and model of equipment covered under this PWS. b. Contractor's Technician shall present a neat appearance and be easily recognized as a contract employee. At the completion of work being performed, the contractor shall be responsible for cleaning up the work area and equipment. All accumulated rubbish shall be removed and disposed of in the appropriate waste receptacles. 8.TEST EQUIPMENT The Contractor shall provide upon request by the POC or designee a copy of the current Calibration Certification of all test equipment which is to be used by the Contractor to perform service under this contract. Calibration of equipment shall be traceable and in conformance with test equipment Original Equipment Manufacturer standards. 9.REPORTING REQUIREMENTS The Contractor shall log in with the POC or designee prior to any services. When the service is completed, the Technician shall document services rendered on a legible Electronic Service Report (ESR). All ESRs shall be submitted to the equipment user for an "acceptance signature" and then to the POC or designee for an "authorization signature". The Technician shall log out with the POC or designee. 10.ADDITIONAL CHARGES There will be no additional charge for time spent at the site during or after the normal coverage area awaiting the arrival of an additional Technician and/or delivery of parts. 11.QUALITY ASSURANCE The government will evaluate the contractor's performance under this contract. For those tasks listed in this PWS, the POC will follow the methods of surveillance specified in this contract. When poor performance is noted, the POC will contact the Contractor's Technician to resolve the issue. If the issue cannot be resolved, the POC will document the observation and forward to the Contracting Officer for action. The Contracting Officer will notify the Contractor of the issue in writing. The Contractor will have 24 hours to correct the issue or explain to the Contracting Officer of any delays. 12.QUALITY SURVEILLANCE PLAN Required ServiceStandardMethod of SurveillanceFactors to consider for Option Year Renewal Emergency Repair Contractor shall respond within 24 hours upon notification.Respond within 24 hours of notification.100% - Each call.Yes Emergency Repair Contractor has required parts on hand for repairs to be completed within 3 days. Contractor shall provide service reports for all preventive maintenance and repairs.100% - Each call. Yes 13.VA REGULATIONS All Contractor employees shall wear visible identification at all times when on premises of the Salt Lake City VAMC. Contractor personnel are responsible for parking in appropriate designated parking areas. The VA will not be responsible for parking violations incurred by the contractor. Information on parking is available from the VA Police or the Biomedical Engineering Department staff. Smoking is prohibited inside any VA facility. Contractors shall adhere to signs posted at the VAMC relating to this facilities public safety requirements. Possession of firearms is prohibited. Enclosed containers, including tool boxes, may be subject to search at the discretion of the VAMC. 14.INFECTION CONTROL The Contractor shall conform to the infection control measures specified by the Salt Lake City VAMC and manufacturers preventive maintenance procedures for the equipment. Preventive Maintenance service will normally be performed in the work area and is subject to infection control procedures.
