- Pursuant to the authority of FAR 13.106-1(b) (1) (i), Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available. The Department of Health and Human Services (HHS) Substance Abuse and Mental Health Services Administration (SAMHSA) Center for Substance Abuse Treatment (CSAT) is looking for a contractor to provide clinical technical assistance in clinical training and clinical mentoring to physicians in all new and existing (60) Methadone clinics within the national pilot Methadone Maintenance Treatment (MMT) program; to assist the Government of Vietnam and relevant ministries in developing a workforce for addiction treatment. The Department of Health and Human Services (DHHS), Program Support Center (PSC), Acquisition Management Services (AMS), on behalf of the Substance Abuse and Mental Health Services Administration (SAMHSA) intends to sole source to: Hennepin Faculty Associates; Minneapolis, MN. USG PEPFAR programming in Vietnam, of which SAMHSA is an implementing partner, has centered on prevention, care and treatment of injection drug use (IDU), since IDU is the leading cause of HIV transmission in Vietnam. The Government of Vietnam's previous approach to addressing injection drug use has been through the criminal justice setting. This latter approach has effectively concentrated the HIV epidemic within penal treatment settings, provided minimal treatment for substance use disorders, and has resulted in an approximate 90% return to drug use upon release from incarceration. The Government of Vietnam has also expressed a desire to continue to find protocols to address a number of issues regarding the provision of effective and evidence based substance abuse treatment and recovery services for the substance abuse dependent population. With PEPFAR and other international support, the Government of Vietnam has adopted methadone maintenance programs for the treatment of opiate addiction. They have done this without any history of medication-based treatment experience or treatment capacity. The intended technical assistance contract is required to develop the needed treatment capacity within Vietnam. This requirement is best fulfilled through SAMHSA and its contracted technical experts, as traditional PEPFAR partners such as CDC, USAID, DOD lack direct expertise in the development and implementation of treatment services as well as clinical training for the treatment of substance use disorders. A national substance abuse treatment curriculum has been developed and an advanced training curriculum for life-long learning is in development. There is an ongoing need for development of further advanced curricula based on (1) the surveyed needs of physicians currently providing methadone maintenance treatment and (2) input from technical experts on observed gaps in knowledge. An initial cadre of Vietnamese physicians has been selected by national and provincial authorities to become mentors and trainers so that training can eventually be provided by Vietnam. This initial cadre of mentors is still in need of clinical supervision and training. The mentors are skilled in the national substance abuse treatment curriculum developed by technical experts, but they continue to need assistance in the development of critical evaluation of clinical scenarios and skills. The needed clinical training includes site visits of methadone clinics to provide training of physician in the care and treatment of IDU prior to and during methadone maintenance treatment as well as team case management. New methadone treatment clinic openings are expected to increase the total to 60 Methadone Clinics and this expansion of services will require direct clinical supervision and teaching at the newly established clinical sites. Hanoi Medical School, in partnership with SAMHSA, has recently created the HIV/Addiction Technology Transfer Center to support future trainings and education of treatment program staff. The technology center will need technical expert input on future curriculum development and clinical supervision of newly trained staff. As arranged by the SAMHSA Treatment Advisor PEPFAR Vietnam, the Contractor will visit Vietnam in three- 2 week intervals and two- 2 month periods from October 2014 to September 2014. This is not a solicitation for competitive proposal. No solicitation document is available.
