Description
DESCRIPTION/SPECS/WORK STATEMENT 1. SERVICES PROVIDED: The contractor shall provide to the Department of Veterans Affairs, Memphis, TN ultrasound procedures. The contractor shall provide services on an emergency basis. Upon request for the ultrasound services, the contractor will also be provided with the term of services required. The Department of Veterans Affairs Medical Center Chief of Staff Service will have final technical and administrative direction over all services provided. 2. General Description: The Department of Veterans Affairs, Memphis will transport patients to contractor for ultrasound examinations. Estimated number of cases annually is 112. Patient transportation will be coordinated by the VAMC emergency room personnel. Emergency services are to be provided after hours Monday through Friday (4:30pm to 8:00am), Saturday, Sundays and Holidays. 3. The Contractor will provide the following: a.Provide ultrasound services within 2 hours of a request from VAMC. The time allowed will include the time needed for transportation to the contractor, performing the exam, and providing a written or phoned report by a radiologist. The 2 hour turn-around-time does not include the time required to transport the patient back to VAMC. Because of the necessity for rapid provision of care, travel time each way to the contractor must not be expected to exceed 30 minutes. b.Critical values (clot, torsion, cholecystitis) will be communicated provider to provider by phone consistent with the ACR guidelines for communication. c.Noncritical values may be faxed at completion of dictation. Exam reports must be faxed over within 1 hour of exam completion. d. Results shall be faxed to the Medical Administrative Assistant (MAA) in the Emergency Department. Plan for Monitoring and Reviewing Quality of Care Provided: a.All services provided by the contractor will be properly documented appropriately (radiological interpretations, images, etc..) to be forwarded to VAMC for review and inclusion into patients VAMC medical record. b.VAMC and contractor will monitor the outcomes of care provided and goals attained. 5. APPROVAL OF CONTRACTOR PERSONNEL: The Department of Veterans Affairs reserves the right to refuse employment under this contract or require dismissal from contract work of any contractor employee or subcontractor employee because of unsatisfactory performance or for any other reasonable reason considered by the Chief, Radiology Service, the Chief, of Staff, and the Contracting Officer to be objectionable, upon written notification of unsatisfactory performance to the Contractor. SPECIAL CONTRACT REQUIREMENTS The contractor agrees in accordance with the terms and conditions stated herein, to furnish to and at the Department of Veterans Affairs, herein called VA, 1030 Jefferson Avenue, Memphis, TN 38104, the services specified and at the prices specified in Supplies or Services section of this contract. The initial listings of services to be furnished shall be identified in Supplies/Services and Specifications/Work statement. 1. SERVICES: a. The services specified in Supplies/Services, Specifications/Work statement, and Special Contract Requirements may be changed by written modification to this contract. The VA contracting officer will prepare the modification. b. Other necessary personnel for the operation of the services contracted for at the VA will be provided by VA at levels mutually agreed upon which are compatible with the safety of the patient and personnel and with quality medical care programming. 2. TERM OF CONTRACT: This contract is will be proposed for 1 base year with annual options years thereafter. The contract is subject to the availability of VA funds. The contracting officer reserves the right to exercise. No services shall be performed by the contractor after exercised option years unless the contracting officer authorizes such services in writing. 3. QUALIFICATIONS: a. Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be subject to review by the Chief of Staff and approval by the VA facility director. 6. CERTIFICATION: Technologists performing the examinations must be registered or eligible for registry by the American Registry for Diagnostic Medical Sonography (ARDMS) OR credentialed to perform the examination at a JACHO accredited hospital.
