Attachment 06 - KNF Food Storage Order Attachment 05 - Experience Questionnaire Attachment 04 - Wage Determination Attachment 03 - FP-03 Sup.Specs Attachment 02 - Drawings Gray Creek Pipe Attachment 01 - Drawings, Pink Creek Culvert Solicitation North End Culverts 2014 C-1 - PROJECT DECRIPTION AND LOCATION (a) Project Description. This project consists of cold milling 160' of existing pavement, hauling cold mill material to government pit, placing and compacting 25 cy government furnished gravel base where needed on road shoulder and placing 75 tons of hot asphalt in 3" overlay. Contractor will provide contract tonnage of asphalt, verifying tonnage by weight tickets on each load. Contractor will prepare and pave road as marked. Contractor will notify the Forest Service 72 hours prior to any paving operations so that locally impacted users may be notified. Contractor will be allowed to work Monday thru Friday, with typical work hours between 7AM and 7PM. Contractor may ask for a variance in hours from the Contracting Officer. No work will be performed on Holidays. Contractor is required to control traffic per MUTCD Standards with flaggers when necessary. Campground and boatramp area will be closed during paving operations. Closures will only be permitted Monday through Thursday and for a maximum of 4 days. (b) Project Location. Project is located approximately 22 miles Southwest of Eureka, Montana. From Eureka Ranger Station travel north on highway 93 approximately.5 miles to the junction of Highway 37. Follow Highway 37 approximately 21.5 miles to the turnoff for Peck Gulch Campground and Boatramp. (c) Pre-Bid Conference and Tour. A pre-bid conference or tour will not be conducted. Prospective bidders are encouraged to visit the project site prior to submitting a bid. (d) Start Work. It is estimated that the Notice to Proceed will be issued about May 11, 2015. (e) Period of Performance. The Contractor shall complete the entire project ready for use not later than 4 days after issuance of Notice to Proceed. The time stated for completion shall include final cleanup of the premises. (f) Price Range. Below $25,000
