Attachment 05 - KNF Food Storage Order Attachment 04 - Experience Questionnaire Attachment 03 - Wage Determination Attachment 02 - Drawings Attachement 01 - Project Map Solicitation 1.1 DESCRIPTION OF WORK A. Summary: The contractor shall furnish all labor, equipment, transportation, supervision, operating supplies, materials and incidentals, unless noted otherwise, to repair the damage caused by the tree that fell on the Sylvanite tree wrapping shed. 1.2 PROJECT LOCATION A. The project is located at the Sylvanite Work Center. B. Directions: From Troy, MT proceed west on Highway 2 approximately 10 miles to the junction with the Yaak River Road (508). Turn right on 508 and proceed 12 miles to the Sylvanite Work Center (on your right). The tree cooler is located down by the river - there is a detailed map in the contract package 1.3 PROSECUTION AND PROGRESS A. The estimated start work date is October 1, 2014. B. There will not be a pre-bid tour for this project. Prospective bidders are encouraged to visit the site prior to submitting a bid. C. Prior to start work a prework conference will be held. D. All work under this contract to be completed by November 1, 2014. E. Submittals: 1. The contractor shall submit the following along with the price quote: a. Safety Plan, including a Fall Protection Plan 2. The contractor shall submit the following at the prework meeting: a. A proposed work schedule b. A list of all subcontractors, and their intended use. F. Weekend and Holiday Work Restrictions: No on-site work shall be performed on weekend or federal holidays unless otherwise authorized by the CO. 1.4 CODES, STANDARDS, AND PERMITS A. Comply with these specifications, project drawings, and all applicable local, State, and National laws, codes, standards, and regulations. Comply with all health and safety codes for personnel and public. 1.5 USE OF PREMISES A. Toilet facilities are not available at the work site. B. Electrical Power is available at the site. 1. Contractor shall supply all power tools and extension cords as needed for the work. C. Water 1. Potable water is NOT available at the work site. The contractor is responsible for providing all water required to complete this project, drinking or otherwise. D. Camping will be allowed at the worksite in areas designated by the CO during periods of active work. Coordinate with the CO a minimum of 14 days prior to start of work. E. The contractor may store materials in areas approved by the CO. The contractor shall be responsible for materials and equipment stored at the site. F. The contractor shall maintain the project area in a neat, orderly, and safe condition at all times. All construction debris and other waste to be cleaned up at the end of each working day and placed in suitable containers. G. Disposal of debris: 1. Dispose of all material removed by this project that is not designated for reuse, salvage, or disposal by Forest Service. 2. Burning of waste materials is not permitted at the project site. 3. Dispose of waste material in a legal manner off of Forest Service property in accordance with Local, State, and Federal Regulations. 1.6 PROTECTION OF PROPERTY A. The contractor shall conduct his work in a manner that will protect the existing property at the site as set forth under Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvement.
