- FBO DESCRIPTION This is a Sources Sought Notice and not a formal solicitation for quotes or proposals. Contractor shall provide all necessary labor, material, equipment, supplies and supervision to provide bread and bakery items in accordance with the terms and conditions of this contract. The Contractor shall provide bread and bakery items to meet the highest customer service standards of the industry in support of the VA Medical Center in Albany NY, identified as Place of Performance this notice. Contract period is October 1, 2014 - September 30, 2015. The option to extend for an additional year is available. All items delivered shall be in accordance with the latest Federal Hospital Subsistence Guide. All deliveries shall conform in every respect to the provisions of the Federal Food, Drug and Cosmetic Act and regulations promulgated thereunder. Request for solicitation notices are to be sent to Edward Lyke, Contracting Officer, at Edward.Lyke@va.gov This Sources Sought Notice will close at 4:00pm on Monday, August 18, 2014. ? The government is contemplating award of a requirements contract. Quantities of items listed in below are estimated quantities only. The government is not committing itself to a minimum quantity of supplies nor is it committing to a maximum. All items listed herein will be purchased exclusively from the successful offeror from the date of contract award through September 30, 2015. This contract extends only to the items used by the Food and Nutrition Service of the Albany, New York VA Medical Center. Any same or similar items supplied to the Veterans Canteen Service of the Albany VAMC are not subject to the terms and conditions of this contract. When considering your capability to provide the required items listed below, use the matrix in determining your total cost for the requirement. This matrix will be included with the solicitation; failure to complete these sections will result in a proposal being deemed nonresponsive. Please note that the matrix below is for the entire FY; at the time of responding to a solicitation, you may wish to submit a pricing matrix broken down into quarterly deliveries/prices. ? DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK DESCRIPTION OF SUBJECT WORK: The Contractor shall provide bread and bakery products to meet the highest customer service standards of the industry in support of the Albany, VA Medical Center (VAMC). PROCEDURES FOR PROVIDING SERVICES: Contractor shall provide all necessary labor, material, equipment, supplies and supervision to provide bread and bakery items in accordance with the terms and conditions of this contract. DELIVERY ORDERS: A.VAMC personnel designated at time of award will place delivery orders. B.VAMC will issue individual delivery orders (purchase orders) for their service. Delivery orders will be picked up at time of delivery. Advance delivery orders will be issued for the first deliveries. DELIVERY: A.All items shall be delivered FOB Destination to the Albany VA Medical Center (VAMC). B.All items delivered shall be in accordance with the latest Federal Hospital Subsistence Guide. C.All deliveries shall conform in every respect to the provisions of the Federal Food, Drug and Cosmetic Act and regulations promulgated there under. D.All bread/bakery items shall be labeled, packaged, and packed in commercially acceptable containers. Where applicable, bread, fresh, type X shall be labeled very low sodium. E.Production of bread shall not precede delivery by more than twenty-four hours. Bread/Bakery items must be freshness dated in the following method: Production date or freshness expiration date directly marked on the packaging or twist tie/plastic closure. INSPECTION AND ACCEPTANCE: A.A point of contact from VAMC will be designated to accept or reject delivery as to quality and condition of bakery and bread items. Items rejected all or in part by the VAMC shall be removed from the premises within 48 hours of notification of rejection. B.Vendor-owned delivery trays, racks, wheeled carts, etc. shall be in clean condition at delivery time; if they are not, vendor personnel will be required to transfer bread products to government owned holding racks. Vendor-owned trays/racks, wheeled carts, etc. utilized in delivery will be held for pick-up by the vendor for reuse. C.Unless offeror certifies that acceptable inspection has been made (any agency of the Federal Government or the AIB International Department of Food Safety/Hygiene) a pre-award onsite evaluation of the offeror's facility will be made. This will include inspection and evaluation of the plant, personnel, equipment and processes, as applicable, of the prospective contractor and conditions found will be considered in award of the contract. The VAMC reserves the right to perform an inspection of the contractor's facility at any time during the contract period and, if it is found that the contractor is not complying with specifications, deliveries will be rejected and the items covered by this contract will be procured in the open market. Any excess cost occasioned by this action shall be charged against the contractor's account. SANITATION AND WORKMANSHIP: Items shall be prepared, processed and packed under sanitary conditions and in accordance with good commercial practice. All containers shall be clean, sound and securely covered or sealed to provide adequate protection from dirt, filth, or other contamination.
