- ATTACHMENT THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This is not a Request for Proposal (RFP) or Request for Quotation (RFQ). In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center located at 4212 N. 16th Street, Phoenix, AZ 85016 is conducting a market survey to identify contractors the following: This is a Radiology Equipment Physic Survey service requirement for the U.S. Public Health Service, Phoenix Indian Medical Center (PIMC). The Phoenix Indian Medical Center facility is located in Phoenix, Arizona. The scope of this service is to provide Medical Physics that diagnostic quality of imaging and safety of medical image equipment, Radiology Unit to meet the manufactory specification requirements, NFPA 101, American College of Radiology (ACR) and Environment of Care (EC.02.04.01) Joint Commission (JC) Standard. The equipment location is Phoenix Indian Medical Center (PIMC), Phoenix, Arizona. The contractor shall ensure each and all physic survey will meet ACR and TJC standards. The overall contract is to help provide quality and safety patient care standards for Phoenix Indian Medical Center.(Refer to attachment A for additional information.) Interested contractors who meet the criteria above may submit information to the acquisitions department. This RFI is open from August 8, 2014 through August 15, 2014. This RFI is for planning purposes only. This sources sought notice/(RFI) is not to be construed as a commitment by the Government, nor will the Government pay for any information solicited.
