- SOURCES SOUGHT Long Term Nursing Home Care This is a Sources Sought Notice/Request for information for planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA does not intend to award a contract on the basis of responses received or otherwise pay for the preparation of any information submitted. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. The purpose of this Sources Sought Notice is to perform market research to determine the existence of businesses that may be interested in and capable of providing long-term nursing home care to qualified veteran patients throughout the state of New Mexico, west Texas, and southern Colorado. This Sources Sought is for information and planning purposes and is not to be construed as a commitment by the government but only as a means of identifying responsible business concerns, registered with the federal government's System for Award management (SAM) https://www.sam.gov/portal/SAM/#1 that are interested in and capable of performing the work described herein. As a result of this Sources Sought Notice, the VA intends to issue a Request for Proposals (RFP) through FedBizOpps for the service described herein. THERE IS NO SOLICITATION AVAILABLE AT THIS TIME. However, should such a requirement materialize, no response or information submitted to and received by the government shall be used to constitute basis for claim against the VA for the resultant use of such information as either part of our evaluation process or in developing specifications for any subsequent requirement. The VA Community Nursing Home (CNH) Program is designed to assist Veterans and their families in making the transition from an episode of hospital care, nursing home care, or domiciliary care to the community. The primary goal of the CNH program is to maintain or restore the Veteran to the highest attainable level of health and well-being. A break in the continuity of care to our Veterans would prove detrimental and negatively impact the wellbeing of the aged Veteran population who require assisted services and a supportive environment. Re-competing the services would result in a change in services to the existing Veterans who have already been enrolled and have been receiving service based on medical necessity for years. A change in vendor may lead to a decline in function or an exacerbation in the level of mental, emotional, or physical function. Consistency in therapeutic milieu is warranted. The primary goal is to maintain or restore the Veteran to the highest attainable level of health and well-being, thus Veteran treatment should be focused on stabilizing symptoms as much as possible. The CNH program has two cornerstones: some level of patient choice in choosing a nursing home in close proximity to their home and family and an approach to local oversight of CNHs consisting of annual reviews and monthly patient visits. Following this approach, the NMVAHCS seeks to establish multiple Basic Ordering Agreements (BOAs) in various areas throughout New Mexico, west Texas, and southern Colorado. This entails placement of the Veteran in a facility that is in proximity to the Veteran's home or place of residence or in proximity to the Veteran's family or social connections. Contracts and agreements under the referenced authority carry out the essential purpose of providing nursing home services to the Veteran and in the Veteran's social community. The New Mexico VA Healthcare System (NMVAHCS) is seeking capability statements from organizations who can provide the required labor, facilities, materials and supplies including private rooms, meal and nutritional services (such as bathing, housekeeping, medication dispensing, laundry, etc.) to provide long term nursing home care for qualified veteran patients throughout the state of New Mexico, west Texas, and southern Colorado. The Government intends to issue an open and continuous Request for Proposals (RFP) and award multiple Basic Ordering Agreements (BOAs) resulting from responses to this Sources Sought. The North American Industry Classification System (NAICS) code for this requirement is 632110 and the small business standard is $25.5M. Potential sources/interested parties are requested to provide a description of their current capabilities, included but not limited to current space available, amenities and/or demonstrate plans to obtain the resources necessary to provide these requirements. Interested parties are requested to provide the following information: (1)Company name, a.Point of contact, b.Physical address, c.E-mail address, d.Telephone and facsimile numbers e.Type and business size; (2) Current and anticipated space available; (3) Amenities and services currently provided or planned to provide; (4) Age of the facility; (5) Name of principles to contact, including title and phone number; (6) Current price list. Documentation should include: (1)Both Government and commercial contracts, (2)References (names, telephone numbers and e-mail addresses) and (3)any other information serving to document the organization's capability. The submission must also include the business entity's: (1)DUNS Number, (2)System for Award Management (SAM) registration information, NOTE: Any potential respondent who is not registered in SAM is invited to submit a response, but must be prepared to initiate and complete registration in SAM immediately.
