- Contract Clauses and Provisions Performance Requirement Summary Performance Work Statement (PWS) (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 14-235-SOL-00022 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses. (iv) This acquisition is a total small business set aside. (v) There is only one contract line item which is to provide Diabetes Education Professional Consultant services in accordance to the Performance Work Statement (PWS) (vi) The contractor shall provide all services required in the PWS. (vii) The period of performance is one year from date of award. (viii) The attached Clauses and Provisions apply to this acquisition. (ix) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer (attached).
