Description
TJCC FAR Clauses Terms and Conditions W-9: Must be returned with Bid Vendor Acknowledge Form-Must be returned with Bid Culinary Food Bid Spreadsheet-Save in Excel format to return to TJCC Cafeteria Food Bid Spreadsheet-Save in Excel format to return to TJCC RFP-RFQ-SOW "This is a Subcontracting Opportunity" Turner Job Corps Center (TJCC) and Education & Training Resources (ETR); located at 2000 Schilling Ave, Albany, GA. 31705 are desirous of a fully insured and licensed food item supplier to submit a bid on the federally funded project mentioned herein; and if successfully awarded the subject TJCC Blanket Food Item Agreement, to satisfactorily perform the following work project according to the established criteria contained within same. B idders must have a "Dun & Bradstreet" (DUNS) number to be awarded a federal contract and provide documentation of such. Bidders must be registered in "System for Award Management" (SAM) before being awarded a federal contract and provide documentation of such. Final complete bids are due to the center, not later than COB August 22nd 2014 at 4:00 PM. Point of contact for this solicitation is the Purchasing Department. They can be reached via email at gail.morman@etrky.com and melody.kersh@etrky.com ALL POTENTIAL VENDORS PLEASE READ AND COMPLY TO COMPLETE SCOPE OF WORK FOR PROPER BID PROPOSAL CONSIDERATION. Scope of Work 1. Vendors are needed to provide and deliver cafeteria and culinary items for the 732 +/- students of the Turner Job Corps Center. 2. All interested vendors must completely fill out, sign and return the "TJCC-2014- Cafeteria and Culinary Supply Item Bid Information Sheet" found on page two of this scope of work with their proposal. 3. All vendors must use the TJCC-2014- Cafeteria and Culinary Food Supply Item Bid Sheets October 1 st 2014 - March 31st 2015" for individual pricing of all Cafeteria and Culinary Supply Items, Bidders MUST submit bid sheets electronically in the provided Excel format as a separate excel attachment (Not PDF format) to : gail.morman @etrky.com and melody.kersh@etrky.com. All other required items and originals can be submitted by mail or sent pdf format electronically. 4. All item brands expected are noted on the bid sheet. If substitute equivalent quality brands are bid, they should be noted with exact details in the additional comment column on the attached bid sheet. 5. All cafeteria and culinary supply items are to be delivered to our location at, 2000 Schilling Ave, Albany, GA. 31705 by the successful bidder (s). 6. All deposits for any item must be included in the unit price. A separate charge for deposits will not be honored. 7. Any expected Fuel/Delivery/Freight/ or Minimum order surcharges must be clearly noted within the item supply proposal. 8. S election of the vendor (s) will be based on the "Best Value Evaluation Panel Review" for TJCC and lowest Fee-For-goods pricing for each item, from vendors that comply with this scope of work. This is NOT single source procurement. 9. Please see attached link for obtaining a DUNS number and registering with the SAM.GOV. http://fedgov.dnb.com/webform https://www.sam.gov/portal/public/SAM/ 10. 11. Please quote items that are routinely stocked by your company, or that can be delivered in our normal delivery scheduled (5 days). 12. Please only quote items that you can supply for the duration of the period of service. Any Items you cannot bid place a NA in the pricing column by that item. 13. Any food item substitution of a product on a delivery must be at the original accepted bid price, and must be a product that is of equal or greater quality. 14. ONLY WEDNESDAY DELIVERIES between 8 A.M and 4 p.m. will be accepted. Make sure your company drivers are aware. 15. Do not ship any "back orders" for items that did not ship with original order. All orders are fill or kill orders. 16. All Bids must include a copy of your current business license), current W9 & our TJCC supplied vendor acknowledgement form- All forms completely filled out and signed (available as a downloadable form).
