- Attachment A SOW for Solicitation 14-100-SOL-00012 for Acute Care Management Model Combined Synopsis and Solicitation 14-100-SOL-00012 for Acute Care Management Model Combined Synopsis/Solicitation 14-100-SOL-00012 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 14-100-SOL-00012 is issued as a request for quotation (RFQ). The Government intends to acquire a commercial item using FAR Part 12 and the Simplified Acquisition Procedures set forth in FAR Part 13. The objective of this acquisition is to develop a conceptual model for the management of acute unscheduled care in the US that integrates the emergency care system into the broader healthcare delivery system, and to identify potential policy solutions that will result in a more patient and community centered system of care. This model will address daily surge control and develop scalable methods to facilitate the efficient response of the acute care system during disasters and public health emergencies. A key goal is to describe how acute conditions and acute exacerbations of chronic conditions should be managed in a patient centered health care system. The project also seeks to understand the role of health care coalitions in the cooperative management of day-to-day acute unscheduled care and during disasters and public health emergences (such as pandemic influenza). This RFQ is issued under NAICS 541618 -- Professional, Scientific, and Technical Services-- Other Management Consulting Services, the size standard is $14,000,000.00. The Government intends to award a firm-fixed price purchase order to the responsible quoter whose offer conforming to this combined synopsis/solicitation will be most advantageous to the Government. This notice neither obligates the Government to award a contract, nor does it obligate the Government to pay for any quote/quote preparation costs. Quotes must be for all items stated. Quotations shall be valid for a minimum of 60 days. Only firm-fixed price offers will be evaluated. The applicable clauses/provisions are those in effect through Federal Acquisition Circular (FAC) 2005-74, effective 01 July 2014 and includes FAC 2005-75 effective 24 June 2014. The combined synopsis/solicitation incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make the full text available. Also, full text of a clause may be accessed electronically at this/these address(es): http://www.arnet.gov. The Statement of Work (SOW) is included as Attachment #A to the Solicitation and shall be used to assemble the quote.
