Description

THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Department of Veterans Affairs (VA) Grand Junction VA Medical Center (GRJVAMC), located in Grand Junction, Colorado is performing market research to determine if there are any qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) Small Business, and (4) Large Business Contractors beside the know sources that we have already identified to provide Intermediate and Standard Complexity Level Surgeries (ISCLS) Services at their accredited Healthcare facility in Grand Junction, CO. The Contractor's facility must ISCLS accredited and be within a fifteen (15) mile geographical radius of the GRJVAMC. There will be several privileged ( at the Non-VA facility) GRJVAMC Surgeons will be performing ISCLS for referred eligible veterans at this non-VA, non-Affiliate facility. Also the contractor shall provide operating room(s) with all required equipment, operation and recovery room staff, and anesthesiology services that are required to perform successful ISCLSs at their primary business location in support of GRJVAMC. This procurement action shall be a Fixed-Price Indefinite Delivery Indefinite Quantity (IDIQ) contract. The Government will use responses to this notice to make the appropriate acquisition decisions. The proposed solicitation will be issued in accordance with FAR Parts 12 and 15. The contemplated total contract period shall be for one (1) base year with four (4) one year option periods not to exceed five (5) years and six (6) months. The total contract amount, including all options years, shall not exceed $ 1,758,000.00. The Selection Criteria include in descending order of importance: Offeror's Implementation / Qualifications; Regulatory Practices including Compliance with HIPAA/OSHA & Department of Veterans Affairs Policies & Procedures; Quality Assurance; Past Performance. All interested businesses should respond by email, with information describing their interest, capabilities, and indicate which business category for which they qualify to the Contracting Officer shown above no later than 4:00 p.m. (Mountain Standard Time), Wednesday, August 08, 2014. Solicitation VA259-14-R-0405 has been assigned and should be referenced on all correspondence regarding this announcement. THIS IS NOT A REQUEST FOR PROPOSAL. The NAICS Code 622110 is applicable to this acquisition, with a Business Size Standard of $35.5 million. Interested contractors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System Award Management (SAM) database prior to award of a contract.
