- This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and quotes are being requested. A written solicitation will not be issued. The solicitation number is VA248-14-Q-1463 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The associated North American Industrial Classification System (NAICS) code for this procurement is 621511 with a small business size standard of $32M. This solicitation starts 07/30/2014 and will end 08/13/2014 at 16:00 pm EST. This solicitation is a Meet or Exceed and is set aside for small businesses. The Department of Veterans Affairs Bay Pines Healthcare system, C.W. Young VAMC, hereafter referred to as CWYVAMC, 10,000 Bay Pines Blvd. St. Petersburg, FL requires technical and professional services for performance and interpretation of specimens, slides, blocks, or images, as ordered, for testing not performed at C. W. Young VA Medical Center. This is a Firm Fixed Price, Indefinite Delivery Indefinite Quantity (IDIQ) contract, Base Year with four (4) one year options requirement. Period of Performance Base Year: 1 Sep 2014 - 31 Aug 2015. SCOPE OF WORK: C. W. Young VA Medical Center at Bay Pines VA Healthcare System seeks pathology consulting and testing services from a reference laboratory for interpreting specimens, slides, blocks, or images as ordered by the pathologists for testing not performed at C. W. Young VA Medical Center. The reference laboratory shall provide expert consultation and second opinion in cases involving cancer patients. These services will aid in the diagnosis and appropriate management in the prevention and treatment of cancer. General: The VAHCS requires the reference laboratory to have the capability to perform and interpret specimens, slides, blocks, or images as ordered by the pathologists for testing not performed at C. W. Young VA Medical Center. This solicitation is for an Indefinite Delivery Indefinite Quantity (IDIQ) contract. Quotes shall be quoted as total cost a year for an estimated yearly total: 280ea - various tests. 40ea - consults - ID#62473 and CPT code 88323. 7ea - various test plus one consult The Reference Laboratory shall: 1.Perform and interpret the specimens, slides, blocks, and/or images 2.Be a CAP/CLIA licensed/approved laboratory 3.Provide CWYVAMC a copy of licensure upon request 4.Capable of utilizing multiple techniques, if needed, including but not limited to: special histochemical stains, immunohistochemistry, immunofluorescence, in situ hybridization, flow cytometry and electron microscopy to achieve accurate, reliable results. 5.Provide a test request form with a statement of medical necessity 6.Provide at no additional cost to the patient or physician: telephone counseling/consultation with reference laboratory's pathologist when deemed appropriate 7.Provide preliminary reports within 48 hours and final report within 72 hours of receipt with the exception of special cases. 8.Reports shall include comments that reflect clinical application. References: "The Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy and Security Rules " The Clinical Laboratory Improvement Amendments of 1988. CLIA regulations, Sections 493.1 through 493.865 Delivery: "Reports shall be faxed to C.W. Young VAMC Pathology and Laboratory Medicine Office at 727-319-1075 and shall be followed by a mailed report to C.W. Young VAMC, 10,000 Bay Pines Boulevard, Building 100, Second Floor Lab, Room 2B-110, Bay Pines, Florida 33744. "Pathologist reviews all results and integrates information into diagnosis and treatment. Progress and Compliance: "Consulting reference laboratory shall provide a final, signed, written report that reflects clinical application. "Reports shall be reviewed / proofed by the C.W. Young Pathology Department for approval. "Invoices will be compared to departmental records of specimens submitted and received reports of tests performed to assure accuracy. Notes and Other Information: C.W. Young VAMC Pathologists will evaluate all requests for cancellation or review of previous findings as determined by their professional evaluation of the results Failure to provide the information in the above paragraph shall result in your quote being determined "non-responsive". The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items. Offerors shall meet the Evaluation Criteria identified in the Statement of Work (SOW). 52.212-2, Evaluations-Commercial Items, with addenda: paragraph (a) is filled in with: 1. Technical and sub-factors: -Certified and Licensed approved documentation for Reference Lab. -Schedule of techniques used to interpreting specimens, slides, blocks, or images as ordered by the pathologists. 2. Pass Performance 3. Price Technical and past performance is significantly more important than price. Any sub-factors rated as unacceptable shall be reason for proposals to be rejected. Offeror shall be registered in the System for Award Management (SAM) database before an award can be made to them.
