- Cover Sheet, schedule of items and other related solicitation documents SOLICITATION NO: AG-84N8-S-14-0048 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes/offerors are being requested and a written solicitation will not be issued. The USDA Forest Service, Utah Acquisition Support Center in Salt Lake City, Utah is soliciting quotes/offerors for the following requirement: Janitorial Services for the Dutch John Work Center On the Ashley National Forest The Synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-22 and 2001-23. The following FAR provisions apply to this acquisition: 52.212-1, Instruction to Offerors Commercial items, and 52.212-2, Evaluation of Commercial Items, applies to the acquisition. Offerors shall include a completed copy of FAR provision 52.212-3, Offeror Representations and Certifications Commercial Items, and furnish Offeror's Federal Tax Identification Number, SAM (System for Award Management, DUNS Number. FAR Clauses 52.212-4, Contract Terms and Conditions Commercial Items and 52.212-5, Contract Terms and Conditions Required Implementing Statutes or Executive Orders Commercial Items apply to this acquisition. Please return the pages as indicated on the cover sheet. Offers are due no later than 4:30 p.m. (local time) Wednesday, August 13,2014 and can be emailed to dlucero@fs.fed.us
