- Statement of Work Request for Quote This is a combined synopsis/solicitation for commercial items in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The applicable North American Industrial Classification Code (NAICS Code) is 541990. The Peace Corps is issuing a Request for Quote (RFQ) for a Patient Safety Organization (PSO) to provide Patient Safety Work Product (PSWP) after a sentinel event. The Government anticipates awarding a multiple award-fixed price contract for a period of performance of 5 years. Technical and cost proposals shall be evaluated as a part of a Best Value determination for awarding contracts. All proposals are due no later than 12:00 p.m. EST on August 26, 2014. Proposals must be submitted to Keisha Dawkins at kdawkins@peacecorps.gov. Vijaya Murthy is a secondary point of contact and can be reached at vmurthy@peacecorps.gov. Peace Corps will review all proposals received by the deadline for responsiveness to the technical specifications outlined. Proposals that are submitted late or are incomplete will not be considered. If an Offeror does not follow the instructions set forth in the solicitation, the Offeror's proposal will be considered non-responsive and may be eliminated from further consideration. Offerors are responsible for submitting proposals, and any modifications, revisions or withdraws so as to reach the Government by the date and time specified in the solicitation. Any proposal, modification, revision, or withdrawal of a proposal received after the exact time specified is considered "late" and will not be considered. This notice does not obligate the Government to award a contract, it does not restrict the Government's ultimate approach, nor does it obligate the Government to pay for any proposal preparation costs.
