- Request for Quote Bldg 870 Floor Plan Bldg 859 Floor Plan Dorm 211-3 Floor Plan Dorm 211-2 Floor Plan Dorm 211-1 Floor Plan Bldg 870 Project Sheet Bldg 859 Project Sheet Dorm 211 Project Sheet Statement of Work Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (ii) The solicitation number F3C3ML4178A001 is being issued as a request for quotation. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74. (iv) This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 238330 with a size standard not to exceed $14 million dollars. (v) DORM 211 CLIN 0001 - Remove existing carpet (2,018 SY) CLIN 0002 - Remove vinyl cove base (5,500 LF) CLIN 0003 - Install glue down Gov't furnished Carpet - Contractor furnished glue (2,018 SY) CLIN 0004 - Install Dark Brown 4" vinyl cove base - Contractor furnished cove base and glue (5,500 LF) CLIN 0005 - Install reducer strips - Contractor furnished (720 LF) BLDG 859 CLIN 0006 - Remove existing carpet (1,106 SY) CLIN 0007 - Remove existing wood and/or vinyl cove base (2,329 LF) CLIN 0008 - Install glue down Gov't furnished Carpet - Contractor furnished glue (1,106 SY) CLIN 0009 - Install Brown 4" vinyl cove base - Contractor furnished cove base and glue (2,329 LF) CLIN 0010 - Install reducer strips - Contractor furnished (70 LF) BLDG 870 CLIN 0011 - Remove existing carpet (1,459 SY) CLIN 0012 - Install glue down Gov't furnished Carpet - Contractor furnished glue (1,459 SY) CLIN 0013 - Install Dark Brown 4" vinyl cove base - Contractor furnished cove base and glue (3,691 LF) CLIN 0014 - Install reducer strips - Contractor furnished (66 LF) (vi) See attached Request for Quotation, Statement of Work, Project sheets and Floor Plans for more details. (vii) Place of performance is Minot AFB, ND 58705 and work shall be completed by 31 December 2014. (viii) The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The government will award a firm-fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The other factors considered will be the ability to meet required period of performance. Award shall be based on best value to the government. (x) The government advises Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Offerors are also reminded to complete Offeror Representations and Certifications on-line at https://www.sam.gov/portal/public/SAM/ prior to submitting an offer. (xi) Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xii) Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.204-10, 52.209-6, 10, 52.219-6, 28, 52.222-3, 19, 21, 26, 36, 35, 36, 37, 52.223-18, 52.225-13, 52.232-33, 52.222-41, 42, 43, and 44. (xiii) Additional FAR clauses that apply are: 52.204-7, 9, 13, 52.223-5, 10 and 52.232-40. Applicable DFARS clauses that apply are 252.203-7000, 7002, 252.204-7003, 7004 Alt A, 252.209-7993, 252.223-7006, 252.225-7001, and 252.232-7003. Applicable AFFARS clauses that apply are 5352.201-9101, 5352.223-9001 and 5352.242-9000. (xiv) There is no Defense Priorities and Allocations System (DPAS) rating applicable for this procurement. (xv) A site visit will be conducted on 31 July 2014, 10:00 am CST at Dorm 211 Tanker Trail, Bldg 859 Fighter Road and Bldg 870 Bomber Blvd, Minot AFB, ND. If the contractor plans on attending, the contractor shall inform the Contract Administrator at least one day prior the said date. (xvi) All offerors must submit a quote by 2:30 pm Central Standard Time on Friday, 8 August 2014. Quotes received after that time may not be considered.
