Description
This is a Request for Information (RFI) only. The primary purpose of this notice is market research. Information gained from as a result of this solicitation will be used for planning purposes. The Veterans Integrated Services Network 18 (VISN 18) requirement for the Southern Arizona VA Health Care System (SAVAHCS), 3601 S. 6th Ave, Tucson AZ 85723. VISN 18 is seeking small business sources (i.e. Service Disable Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, 8(a), Woman Owned, Small Disadvantaged, Small Businesses, etc.) to provide VeriQ - Intraoperative TTFM and Doppler velocity measurement device (brand name or equal) for Cardiothoracic Surgery. Statement of Work The coronary arteries are the major vessels that supply the heart with blood. When these arteries become damaged or diseased, usually due to a build-up of fatty deposits called plaques, it is known as coronary artery disease (CAD). These deposits can narrow the coronary arteries causing diminished blood flow to the heart, eventually leading to symptoms such as chest pain, shortness of breath or even a heart attack. CAD is the most common type of heart disease. It is the leading cause of death in the United States in both men and women. During Cardiac surgery, coronary artery bypass graft surgery, CABG, the most commonly performed open heart operation in the United States; blood flow is rerouted through a new artery or vein that is grafted around diseased sections of the coronary arteries to increase blood flow to the heart muscle tissue. Performing a flow measurement after completion of the graft is the quickest and most accurate way to verify graft patency while the patient is still in the operating room. Manufacturer, model no., and salient specifications are listed for reference only; products meeting or exceeding the listed specifications will be considered as equal. Alternate products proposed shall be accompanied by a manufacturer's product specification sheet. Items requested: VeriQC Imaging System 2mm QUICKFIT PROBE 3mm QUICKFIT PROBE 4mm QUICKFIT PROBE 5mm QUICKFIT PROBE 15Mhz IMAGING PROBE This purchase is only for the SAVAHCS CARDIOTHORACIC OPERATING ROOM. This device will measure blood flow of the coronary artery bypass graft postop. Integration Management Control Planning: Provide the technical and functional activities at the required level for integration of all tasks specified within this SOW. Include productivity and management methods such as quality assurance, progress/status reporting and program reviews. Provide the centralized administrative, clerical, documentation and related functions. Contract Management: Prepare a Contract Management Plan describing the technical approach, organizational resources and management controls to be employed to meet the cost, performance and schedule requirements throughout Contract execution. Provide a monthly status report monitoring the quality assurance, progress/status reporting and program review applied to this contract. Deliverables:Contract Management Plan - Monthly Status Report Performance Monitoring: This is a SOW for the Veriq CABG graft postop. The performance of these scopes will be monitored daily and reported to the COR, Kathleen Rutter, RN. Inspection and Acceptance Criteria: Biomed will inspect this device prior to usage. The VERIQ vendor will provide training to the cardiothoracic surgeons and OR team.
