- Wage Determination 05-2215 (Rev. 15) Request for Quote Performance Work Statement, dated 1 July 2014. This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.601(b), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74. This solicitation is issued as a request for quotation IAW FAR Part 12 and 13. Submit written quotes in reference to Request for Quote (RFQ) solicitation number FA4621-14-T-0015. This requirement is a 100% total small business set-aside IAW FAR 19.502-2. The NAICS code for this requirement is 611620, with a small business size standard of $7,000,000. McConnell AFB, KS plans to acquire the following service: Aerobics Instructor The following is a description of the propoosed contract line items (CLINs) as dictated in the attached request for quote: Line Item 0001: Aerobics Instructor The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision, and transportation necessary to conduct indoor cycling, land, and water aerobics instruction in accordance with the performance work statement, dated 1 July 2014. Period of Performance: 1 October 2014 - 30 September 2015 Line Item 1001: Aerobics Instructor The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision, and transportation necessary to conduct indoor cycling, land, and water aerobics instruction in accordance with the performance work statement, dated 1 July 2014. Period of Performance: 1 October 2015 - 30 September 2016 Line Item 2001: Aerobics Instructor The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision, and transportation necessary to conduct indoor cycling, land, and water aerobics instruction in accordance with the performance work statement, dated 1 July 2014. Period of Performance: 1 October 2016 - 30 September 2017 Line Item 3001: Aerobics Instructor The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision, and transportation necessary to conduct indoor cycling, land, and water aerobics instruction in accordance with the performance work statement, dated 1 July 2014. Period of Performance: 1 October 2017 - 30 September 2018 Quote Specifics: DUE DATE: Quotes are due to this office NLT 1:00 PM CST, 5 August 2014 Quotes must be sent via e-mail to A1C Gregory Smith at Gregory.Smith.86@us.af.mil and to SrA Philip Gray at Philip.Gray.2@us.af.mil All quotes must include the following general information: 1. Price Information: Vendor must complete the attached request for quote. Evaluation of Quotes: Award will be made to the responsive offeror determined to be the most advantageous to the government in accordance with the criteria listed in the attached solicitation. FAR 52.217-9, Option to Extend the Term of the Contract (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 4 years. FAR 52.232-18, Availability of Funds (Apr 1984) Funds are not presently available for this contract. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. Contingent upon the availability of funds, the government reserves the right to award all, some, or none of the subject line items and may execute the award in any combination of the line items. The Government obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made.
