Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, under the authority of 38 U.S.C. 8153 and VAAR part 873, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number VA247-14-R-1098 is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-75 effective 24 June 2014. This acquisition is totally set-aside for Service Disabled Veteran Owned Small Business (SDVOSB) and the associated NAICS code is 621111 - Offices of Physicians (except Mental Health Specialists), with a small business size standard of $11M. The Dorn VA Medical Center has an urgent requirement for 1 FTE (approximately 520 hours per quarter) General Surgeon services. Scope of work and specifics of the requirement can be found in the Performance Work Statement (PWS), Attachment 1. The anticipated period of performance is 1 August 2014 thru 31 July 2015, with no option periods. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, with addenda, see Attachment 2. The provision at 52.212-2, Evaluation -- Commercial Items, also applies. The evaluation criteria to be included in paragraph (a) of that provision are: Technical, Past Performance and Price. Technical and past performance, when combined, is more important than price. All offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and all applicable clauses therein. All offers are due to the point of contact no later than 3:00 pm CDT on July 24, 2014. Offers shall be submitted electronically and in the format described in addenda to 52.212-1 (Attachment 2).
