Description
General The VA Medical Center, Canandaigua, New York is engaging in a short-list selection as defined in FAR 36.602-5(a) of an AE (SDVOSB) for a "space study". The VA Medical Center, Canandaigua, New York is seeking profession A/E firms, under NAICS 541310 to complete a "space study". This is NOT a request for proposals. It is anticipated interviews or discussions for selection will be conducted in August 2014. Set-Aside: This procurement is set-aside for Service Disabled Veteran Owned Small Businesses (SDVOSB). The NAICS code for this procurement is 541310; the small business standard is $. SDVOSBs shall be verified in www.vetbiz.gov at time of offer and award and be registered under the applicable NAICS code. Interested SDVOSB firms: "SDVOSB A/E's with a SF-330 on file within the past twelve months are being considered and do not need to re-submit a SF330. Interested firms MUST confirm with the Contracting Officer that their SF330 is on file. "SDVOSBs who have verified they have a SF330 file are welcome to submit supplement information to their existing SF330 for this specific project. Supplements shall be provided if any members/sub consultants are changing. "Other SDVOSB firms having a main or satellite office within a 300 mile (driving)radius from 400 Fort Hill Avenue, Canandaigua, NY 14424 may submit SF330's for consideration to the Contracting Officer. "SF330 must be sent electronically (PDF or Word) to Cheryl Brimmer: Cheryl.brimmer@va.gov. The overall size of the file cannot exceed 8MB; if additional space is required, divide the file into segments with each file size less than 8MB. No more than the first 3 files at 8MB each will be considered. "SF330s must be received by 4:00 pm EST, August 1, 2014. E-mail confirmation will be provided. EXISTING CONDITIONS: The Canandaigua VAMC provides geriatric nursing home care services in four "neighborhoods", located in building 3 A floor, building 7 A floor, building 7 B floor, and Building 8 A floor. Currently, the second floor of building 8, 8B, is underutilized and leadership has determined that the staff on that floor can be re-located. A/E SCOPE OF SERVICES 1.Perform a space study to provide schematic level plans to utilize Building 8 B floor for private patient rooms, in order to make all rooms on 8A private rooms, and as many rooms on 7A and 7B private while maintaining the current census. Included in the scope of this study will be the any construction to maximize the overall number of private rooms by re-partitioning any remaining non-private rooms to achieve the maximum total number of private rooms with a maximum construction budget of $750,000, as well as to provide a schematic level cost estimate and phasing plan. 2.Attend study review meetings and make all necessary site visits during design to gather information necessary from the existing sites, building plans, utility plans, etc. to provide a complete assessment of the proposed work. The completion of the study shall represent the schematic design. At completion, provide the study, associated drawings, and schematic estimate. 3.Utilizing the results of the study, complete a full design, including necessary phasing, to accomplish the study recommendation chosen. 4.Provide an electronic set and one (1) hard copy set of "Architectural E1" (30"x42") size schematic drawings, and cost estimate at the 50% and the 100% review stage. Also, provide an electronic set and one (1) set of drawings and cost estimates at final study submission. Electronic set at 100% final stage is to be provided in both PDF and AutoCAD 2012 formats, signed and sealed by a Licensed, registered Engineer. 5.Project completion of the study and design, including all deliverables, is to be 120 days from award of contract. Evaluation Factors The following evaluation factors will be used when reviewing submitted SF 330 data and will comprise the basis of information to be used by VAMC Canandaigua for A/E evaluation and selection. The evaluation factors are shown below in descending order of importance (highest importance at the top). Firms submitting SF 330 data for consideration must address each of the following factors in the submitted SF 330: 1.Professional Qualifications of Team: Professional qualifications of the team proposed as specifically related to the requirements of this particular project (background of personnel: project manager, key personnel, consultants; project-related professional qualifications; specialized experience (project-related) of team members). Resumes should include SDVOSB Owner, Project Manager, Lead experts in the subject matter; provide current professional registrations/certifications of person/people who will be stamping any drawings if not listed above. 2.Specialized Experience of Proposed Team: Specialized experience of the team proposed for this project (technical competence in the type of work required with specific emphasis on experience related to work at VA Medical Centers). List individual(s) that be doing field investigation on this project and their past experience and knowledge of working with VA Medical Centers within VISN 2. 3.Past Experience/Past Performance: Provide prior prime contractor (SDVOSB) experience on projects (at least three (3) but no more than five (5)) awarded within the last five years for work similar in size, scope and complexity to this project. Include the following information for each project listed: a.Name, Address, Telephone Number, and Fax Number of a customer point of contact (POC) who would have the most detailed information about the AE's performance. b.Project location, award date, original completion date, actual completion date, original contract award amount, final contract amount. Include names, addresses and phone numbers of subcontractors used. c.Provide a narrative indicating lessons learned from these projects that will be helpful or applicable to this design. Indicate how these projects are similar in scope, design and complexity to this design. 4.Proposed Management Plan and Approach: Management plan for the study phase and the technical report philosophy. Include information on how the prime firm will perform 50% or more of the work. 5.Location: Location (proximity of facilities and personnel to the geographical area of the work; corporate knowledge of local conditions related to the locality of the project) 6.Project Control: Capacity to accomplish the work in the required time: assume an October 2014 start date with a requirement to provide the results of the study not later than February 2015. The study will need to be completed in approximately 140 days. 7.Estimating Effectiveness and Project Control: Construction estimate versus construction award amount: provide data/information on the ten most recently completed projects) and Project Control (describe techniques used by the AE to control the schedule and costs and identify personnel responsible for these functions). For estimating effectiveness, include only similar projects in New York State. VA work and hospital work within Upstate New York will be graded higher. Include who is responsible for quality control/ quality assurance of drawings and how that process works with subcontractors. 8.Miscellaneous Experience and Capabilities:, CADD & Other Computer Applications, Value Engineering and Life Cycle Cost Analyses, Environmental and Historic Preservation Considerations, CPM and any other relevant information. 9.Awards and Recommendations: Identify any awards received for design excellence. 10.Insurance and Litigation: Identify the type and amounts of liability insurance carried and who the provider is. Identify litigation involvement over the last five years with resulting outcome.
