Contractor Insurance Notification Wage Determination Statement of Work RFQ Pricing Worksheet Catholic Priest Combined Synopsis Solicitation This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4608-14-Q-S074, in accordance with FAR Parts 12 and 13, and is in accordance with provisions and clauses effective in Federal Acquisition Circular (FAC 05-74 effective 01 July 2014). The North American Industry Classification System (NAICS) number is 813110, SIC 8661, and the small business size standard is $7,500,000.00. The proposed acquisition is set-aside for small businesses. All small businesses are encouraged to participate by submitting quotes. Please see the attached RFQ for further details. The service being procured is: CLIN 0001: (1) CATHOLIC PRIEST NTE 1,560 hours, See Pricing Worksheet and Other Attachments Period of Performance: 01 August 2014 through 31 July 2015 The following FAR references may be accessed from http://farsite.hill.af.mil. The Government will award a contract resulting from this synopsis to the responsible offeror whose offer conforming to the synopsis will be most advantageous to the Government, price and other factors considered. OMBUDSMAN (AUG 2005) (5352.201-9101) (a) An ombudsman has been appointed to consider and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. The ombudsman may not serve as an advocate for any party. (b) Before consulting with an ombudsman, concerned parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employe actions, contests of OMB Circular A-76 competition performance decisions). The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be technically acceptable, and therefore will not be considered. Once an offer has been determined to be technically acceptable, and in compliance with all directions in this solicitation, the Government will award to the offeror with the lowest evaluated price.
