Description
Quotation Submission Form, pricing per each contract line item number (CLIN). Provisions and Clauses, lists those provisions and clauses that are applicable to this procurement. Statement of Work (SOW), sets forth the work efforts required to produce and deliver the required Master Dies and working Hubs for heraldic items that are used by the Marine Corps to duplicate medals, badges and decorations. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement (including attachments) constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This combined synopsis/solicitation number M67854-14-Q-1016 is issued as a Request for Quote (RFQ), for a firm-fixed-price (FFP) type contract. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisitions Circular 2005-74. All responsible sources may submit a quotation in response to this solicitation, which shall be considered. A contract may be awarded resulting from this solicitation to the responsible vendor whose lowest price technically acceptable quotation conforms to the solicitation. The procurement is 100% set aside for Small Business concerns and the associated North American Industrial Classification System (NAICS) code is 332999 and the applicable size standard is 750. Attachment 01, Statement of Work (SOW), sets forth the work efforts required to produce and deliver the required Master Dies and working Hubs for heraldic items that are used by the Marine Corps to duplicate medals, badges and decorations. Attachment 02, Provisions and Clauses, lists those provisions and clauses that are applicable to this procurement. Attachment 03, Quotation Submission Form, pricing per each contract line item number (CLIN). It is the vendor's responsibility to monitor the FBO system for amendments issued in reference to this requirement. All Contractors who provide goods/services to the DOD must be registered in the SYSTEM FOR AWARD MANAGEMENT (SAM) to receive an award, FAR 52.204-7 System for Award Management. Online Certifications and Representations are also required. Failure to comply with the SAM registration may deem a vendor ineligible for award. All written submissions shall be received at the following location no later than 2:00 PM EST July 24, 2014. No telephonic responses or email responses will be accepted. Quotation Preparation Instructions General Quotation Format - Quoter shall submit quotes in accordance with FAR 52.212-1 Instructions and the Addendum to FAR 52.212-1 (See Attachment 02). Total costs per each contract line item number shall be entered on the attached RFQ (See Attachment 03- Quotation Submission Form). Quotes shall have a cover sheet marked with the copy number (e.g. Copy 1 of 2), title, RFQ identification and classification (i.e. M67854-14-Q-1016 Hubs & Dies) and Quoter's name. Be sure to apply all appropriate markings including those prescribed in accordance with FAR 52.215-1(e), Restriction on Disclosure and Use of Data, and 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Quote Information and Source Selection Information. Documents shall be stamped "Source Selection Sensitive - See FAR 2.101 and 3.104." Quotes shall be clear, concise, and logically assembled. Information may be single-spaced. Any font size used shall be large enough to be easily legible with the unaided eye when printed.
