Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9114F-14-T-0048 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 541511. The Europe Regional Contracting Office has a requirement to purchase renewal of Exit-Writer License to include Support maintenance services for pre-installed Exit-Writer.Net-based desktop application at the Landstuhl Regional Medical Center (LRMC) Emergency Department, Landstuhl, Germany. The contract line items (CLINs) for this acquisition are as follows: Line Item 0001 Exit Writer License and Maintenance Support Services, to include updates, upgrades, patch notification, remote technical support and training, to the installed Exit-Writer software at the Landstuhl Regional Medical Center (LRMC) Emergency Department. Base Period: 20 September 2014 thru 19 September 2015. Unit of Measure: Year Qty 1 Line Item 0002 Contractor Manpower Reporting in accordance IAW 6.0 of the PWS. Base Period: 20 September 2014 thru 19 September 2015 Contractor shall complete the CMR report not later than 31 Oct 2015. LINE ITEM 0001 Subfactor 1: TRAINING AND EXPERIENCE. Quoter's Systems/Database Administrators shall have been factory-trained and have a minimum of 2 years-experience, working on technical support, supervision, and other items necessary to properly perform Exit-Writer.Net-based desktop application technical support and maintenance. Subfactor 2: TECHNICAL APPROACH. Quoter's quote shall clearly describe what resources - personnel, materials, equipment and training programs - will be dedicated to meeting the requirements outlined in the PWS, providing support maintenance that will continue to allow current software to be compatible with the Composite Health Care System (CHCS) system Subfactor 3: ENGLISH PROFICIENCY. Quoter's Systems/Database Administrators shall be able to read, write and speak English. Subfactor 4: LICENSES. Quoter's license shall grant a limited, non-exclusive, non-transferable license, to use, to access and to display the content of the Exit Writer application, on LRMC applicable forms and documents, to include text, graphics, databases, code and information available through the application. Factor II - Price. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All CLINs shall have been priced by the vendor. If the vendor failed to price all CLINs, the quote will not be considered for award. Failure to receive an acceptable rating of any of the above Technical Factors will result in an unacceptable technical rating unless the error(s), deficiency (ies) or omission (s) can be corrected. All CLINs shall be priced. If you fail to price all CLINs, your quote will not be considered for award. ADDENDUM TO FAR 52.212-2 The Government will issue a Purchase Order to the quoter whose quote represents the best value to the Government, price and other factors considered. Award will be made on the basis of Lowest Price, Technically Acceptable (LPTA) quote. If any Technical requirement below is rated quote mark Unacceptable, quote mark then the Government will no longer consider issuing a Purchase Order to that quoter, thus the quoter will be ineligible for award. EVALUATION STANDARDS. The quoter's technical quote will be evaluated using the following standards for a final rating: ACCEPTABLE: A quote that offers a service/ product which meets all technical requirements identified in the solicitation. UNACCEPTABLE: A quote that offers a service/product which fails to meet one or more of the technical requirements identified in the solicitation. SPECIAL NOTE: All vendors must be registered in the System for Award Management (SAM) prior to award, and lack of registration shall make an offer ineligible for award.
