- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA257-14-Q-0709. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72,effective April 29, 2014. The associated North American Industrial Classification System Code (NAICS) for this procurement is 339112 with a small business standard of 500 employees. This acquisition is solicited as a set-aside for small business. Network Contracting Office 18 requests responses from qualified sources capable of providing the following item(s): STATEMENT OF WORK NAVAHCS Simulation Center Package 1 1. Purpose: This product is required for NAVAHCS to meet one or more of the following requirements: provide required clinician competencies &/or privileges, meet VHA Directives, Joint Commission requirements, OIG compliance and ensure patient safety and veteran centered care. Simulation training is an essential educational strategy for health care systems to improve patient safety. In completing simulation training through the highest level of real world replication, simulation-based medical education enables knowledge, skills and attitudes to be acquired for all healthcare professionals in a safe, educationally orientated and efficient manner. Simulation equipment is necessary to amplify real-patient experiences with guided experiences, artificially contrived, which evokes or replicates substantial aspects of the real world in a fully interactive manner. 2.Requirements: Foot & Ankle Joint Injection Simulator Equipment requirements are as follows: Properties include the ability to practice soft tissue joint injection, palpation techniques, and injection sites for MTP joint, Morton's neuroma, tarsal tunnel, plantar fascia, sinus tarsi, retrocalcaneal bursa, tibiotalar jt, and subtalar jt. Requirements: Lipoma Pad Equipment requirements are as follows: Properties include two lipomas located within the fat layer. Provides ability to surgically remove lipomas and suture tissue closed. Requirements: Online & Hands On Hybrid Suturing Tutor Equipment requirements are as follows: Self-paced online suture learning modules to teach healthcare providers to suture with skill and confidence. At least one year access to the online course. The following learning modules must be addressed for adequate competency: Suturing techniques, suture aftercare, handling of instruments, incision of skin & subcutaneous tissue, needles & sutures, knot tying, scrubbing, gowning & gloving, cleaning a wound, local anesthesia, patient management & consent, and other methods of wound closure. Must include hands-on suturing kit with instruments, sutures, knot tying kit, skin pad and jig. Must have a PDF course manual with the ability to download. Requirements: Sebaceous Cyst Pad Equipment requirements are as follows: Properties include two cysts per pad positioned immediately below the skin with simulated pus. Requirements: Shoulder Joint Injection Simulator Equipment requirements are as follows: Properties include the ability to practice soft tissue joint injection, palpation techniques, and five anatomical injection sites: subacromial space, acromioclavicular jt, bicipital groove, glenoid fossa anteriorly and posteriorly. Package to include no-trace marker, shoulder needle set, shoulder skin, injection book, shoulder for injection, feedback console, carry case. New equipment Only: No refurbished products. Items must be in original packaging, never used, and not altered in any way Components of requested equipment to includes spreader bar, different size slings, battery must be manufacturer-approved and must not be compatible, remanufactured, or refurbished equipment. All items must be covered manufacturer's warranty and procured through a manufacturer approved distribution channel. Sellers must be able to document their document ability to provide items through manufacturer approved distribution channels upon request. The Seller confirms to have sourced all products submitted in this Bid from manufacturer approved channels for Federal sales, in accordance with all applicable laws and manufacturers' current applicable policies the time of purchase.
