- Request for Quote: RFQ-DC-14-00091 National Hazmat BPA Procurement Office: United States Environmental Protection Agency, Headquarters Procurement Operations Division (3803R) 1200 Pennsylvania Avenue NW, Washington, DC 20004 Response Date: August 7, 2014 5:00pm (Eastern Standard Time) The United States Environmental Protection Agency's Safety, Health and Environmental Management program located in Washington, DC is seeking quotes for the services listed in the attached Statement of Work. Quotes are being requested in accordance with Federal Acquisition Regulation (FAR) Part 13, Simplified Acquisition Procedures. A single award or a multiple award BPA may be issued to the successful contractor or contractors. The anticipated period of performance for the BPA award will be for 5 years (60 months). This RFQ consists of the attachments listed below: Please review each in its entirety as you prepare your response: 1)Standard Form 18 2)Cover Memorandum (RFQ) a.Statement of Work b.Technical Evaluations Factors c.Instructions to Contractors d.Scenario and Labor Categories e.Applicable Clauses 3)Attachment 1 Representations and Certifications 4)Attachment 2 EPA Regional Locations 5)Attachment 3 Client Authorization Letter 6)Attachment 4 Past Performance Questionnaire Quotation packages submitted in response to the RFQ, shall include the following: 1.An Executed copy of the Standard Form 18. Please ensure blocks 12 thru 18 are completed. 2.Interested contractors shall submit a technical proposal when responding to the Scenario in the RFQ, in accordance with the instructions outlined in this document. 3.Prepare a cost estimate by completing the attached Excel spreadsheet under the Scenario. 4.Interested contractors should also submit certifications as identified in Attachment 1. Please submit any technical questions related to this RFQ to Contract Specialist Dionne Wright at wright.dionne@epa.gov and Contracting Officer Claudia Armstrong at armstrong.claudia@epa.gov by July 31st, 2014. Please submit an electronic copy of your quotation package to Contract Specialist Dionne Wright at wright.dionne@epa.gov and Contracting Officer Claudia Armstrong at armstrong.claudia@epa.gov. Quotes must be submitted no later 5 p.m. (Eastern Standard Time) Thursday August 7th, 2014. No telephone inquiries will be accepted. This RFQ and associated attachments are posted to the FedConnect web portal. All current and potential contractors must use FedConnect which is available at https://www.fedconnect.net/FedConnect/. There is no charge to use FedConnect. FedConnect is hosted by Compusearch, vendor/contractor questions relating to FedConnect will be answered by Compusearch, and contact information is provided at the FedConnect web site.
