- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-Q-14-0016. This procurement is 100% small business set-aside under NAICS code 621111 with a standard business size of $10.0M. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74, effective July 1 2014. Contractor shall provide a firm, fixed-price clinic rate for one eight (8) hour clinic, for one year period of service. The Clinton Indian Health Center intends to have at least one clinic per month with an option of two, if needed. Estimated start date is August 1 2014. The closing date for receipt of quotes is July 18, 2014. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on July 18, 2014. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Qualified vendors will review the following and submit applicable information. No phone inquiries. STATEMENT OF WORK A. SERVICES: The contractor shall provide on-site qualified care of orthopedic services at the Clinton Service Unit, Clinton, Oklahoma. The contractor shall furnish services to include, but not limited to the following: 1. Observing and assessing patient's clinical condition; recognizing, identifying and interpreting serious situations and immediately deciding proper action. 2. Formulating a physician diagnosis. 3. Developing a plan of care for individual patients by interviewing patient and family members, reviewing the patient's health record, and determining the kind of health and physical care required, including discharge planning. 4. Initiating appropriate measures in emergency situations. 5. Operating and monitoring general and specialized medical equipment utilized in Ambulatory Units. 6. Prescribing therapeutic measures such as medications, IV fluids, etc. 7. Planning, providing and documenting the teaching of patients and families in preventive, curative and rehabilitative measures. B. REFERENCE MATERIALS: Vendor shall be required to develop familiarity and abide by current policies, procedures, by-laws, and regulation of the clinical and medical staff of the Clinton Indian Health Center. PERIOD OF PERFORMANCE: The period of performance is one year from approximate start date of August 1 2014. D. SPECIAL TERMS AND CONDITIONS: The contract physicians shall have met the following requirements: (1) Rubella immunity must be documented either by serologic test or proof of immunization (2) A yearly PPD is required for all persons who have had no previous positive tuberculin skin test (3) A series of three (3) Hepatitis B immunizations or serologic proof of past infection is required E. SCHEDULE OF WORK: Contractor shall provide one eight (8) hour shift clinic per month as agreed upon between contractor and the Clinton Indian Health Center. There may be two (2) meetings a month, if needed, and also agreed upon between contractor and the Clinton Indian Health Center. F. MINIMUM PROFESSIONAL QUALIFICATIONS: The contractor shall possess the following minimum professional qualifications. 1. Current licensure as a medical doctor in a state of the United States, presented to Clinical Director prior to beginning work. 2. A 2-year, 3-year, 4-year medical school graduate must have a minimum of - one year of physician experience within the last 3 years. G. REPORTING REQUIREMENTS: The contractor shall report to the assigned department to Clinical Director or CEO upon arrival for duty and report off to the oncoming shift before leaving duty. H. GOVERNMENT PROPERTY/SPECIALITY STAFF/SUPPORT STAFF: The Government will provide the facilities, supplies and the equipment of the Clinton Indian Hospital and all hospital and specialty/support staff. I. SERVICES RENDERED STATEMENT The contractor shall submit a copy of its invoice to the Clinton Indian Health Center, Attention: Material Management, Route 1 Box 3060, Clinton, OK 73601. Contractor shall attach copies of the daily time sheets which have been approved by signature of the Clinical Director or authorized designee stating that services were rendered on the invoice copy, which will expedite payment for services. J. OVERSIGHT CONTROLS: The contractor shall provide services directly under the oversight of the assigned Clinical Director. The Director defines the objectives, priorities, deadlines, and informs the contractor about changing priorities, unusual deadlines and objectives. K. INVOICES An original invoice shall be submitted monthly. The contractor shall submit their invoice to: Oklahoma City Area Indian Health Service, Attn: Financial Management, 701 Market Drive, Oklahoma City OK 73114. 1. Contractor's name 2. Invoice date 3. Purchase Order number (shall be assigned) 4. Description of services 5. Quantity of services rendered 6. Cost 7. Payment terms 8. Where practicable, name, title, phone number, and complete mailing address of responsible official to whom payment is to be sent. L. INVOICE PAYMENT: Payment will be made on this contract as close as possible to, but not later than thirty (30) calendar days after an invoice containing the information set forth in the invoice submission article of this contract is received in the payment office designated in this contract. An invoice will be deemed received on the latter of: The date the invoice is actually received in the designated payment office, or the date on which the government accepts the property of services required under this contract. Unless a different time period is stated elsewhere in this contract, inspection/acceptance required by this contract will be completed by the Government not later than five (5) Government working days after the services are rendered. Payment shall be made within thirty (30) days after the last day of the month during which this invoice covers. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) completed "Temporary Waiver of Character Investigation" and "Declaration for Federal Employment" form (attached) (2) two past performance references to include the contact name phone number, contract number, company name, brief description of project (3) registration in the System for Award Management (SAM) database (reference https://www.sam.gov/portal/public/SAM/). The provisions of 52.212-2, Evaluation - Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications; (3) Past Performance. All factors are equal in importance. THE INDIAN HEALTH SERVICE WILL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO RESPOND TO THESE EVALUATION CRITERIA AS FOLLOWS: A) registration in the System for Award Management (SAM) database (reference https://www.sam.gov/portal/public/SAM/. Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database. B) Propose and provide an all-inclusive clinic rate for services. The estimated quantity is twelve (12) eight-hour (8) clinics for a one-year period. This quantity could be increased to twenty-four (24), if the requestor deems it necessary. The estimated start date of award is August 1, 2014. C) Provide a "Temporary Waiver of Character Investigation" and "Declaration for Federal Employment" (attached). D) Certifications in ORCA at https://orca@bpn.gov. E) Current license as a Medical Doctor (any U.S. state or territory). F) Proof of current medical insurance in the appropriate amount of $1,000,000 per occurrence and $3,000,000 aggregate.
