Description
This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 622110 in VA Butler Healthcare geographical catchment area. Under the authority of Public Law 104-262 and 38 USC 8153, the Contractor will be required to provide MRI Radiology Services, with Government Furnished Property in accordance with the terms and conditions stated herein, to VA Butler Healthcare (referred to as the VA), 325 New Castle Road, Butler, PA 16001, for the services and prices specified in the Schedule of Supplies and Services. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws. The standard of care should be equal to that provided by VA Butler Healthcare. All preventive maintenance updates, services, and checks as well as all liability for the equipment will be the responsibility of the Contractor. The contractor must be able to provide the services within a ten (10) minute driving radius of VA Butler Healthcare. DESCRIPTION: The Contractor will be required to perform and interpret MRI/MRA as specified in the Schedule of Supplies/Services, at the Contractor's place of business, for the life of the equipment. All pricing will include performing, interpreting each procedure and be all inclusive (injectables, medications, etc) in accordance with all terms and conditions of the contract. The cost for each procedure will apply to all Veterans referred by a VA Provider at the VA's expense. SCHEDULING: The Contractor is required to schedule, complete, and interpret all procedures within the following timeframes: STATS - The Contractor is required to schedule, complete, and interpret all VA STAT orders within 4 hours of the procedure request. ROUTINE - The Contractor is required to schedule exams within 14 of receipt of referral/authorization by the VA, unless ordered/requested otherwise. The contractor is required to, interpret and report all VA ROUTINE orders within two (2) workdays of completion of exams. NOTIFICATION OF INTERPRETATION: Upon completion of each procedure, the contractor will notify the VA with the interpretation Contractor Physicians are required to be board certified. The Contractor must provide information necessary to credential and privilege all physicians providing services under this contract. All contract physicians will be required to complete this application and provide information to complete this process. Please provide a capability statement with the flowing information. 1. General information of your company name, address, contact person name, phone number, fax number, email address(s) and DUNS number and company website if available. 2. Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged Business, Service Disabled Veteran Owned small business or Veteran owned small business. 3. Describe your facilities experience providing services to the Department of Veterans Affairs? If you have not done business with the government provide your facilities capability of performing MRI procedures and providing the results. Responses to this market survey should be in writing and emailed to andrew.taylor3@va.gov. No telephone responses will be accepted. Responses must be received no later than 3:00 pm July 10, 2014. No solicitation document is available at this time; this notice is to acquire information only. The agreement for this procurement is projected to be sent for the contractor's acceptance in August.
