- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. 1. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research only to make appropriate acquisition decisions and to gain knowledge of potential qualified Large and Small Businesses interested and capable of performing the work. Interested Contractors should respond to this notice by July 14, 2014 by providing the following via email only to Sandra.Fusco@va.gov : (a) company name (b) address (c) point of contact (d) phone and email of primary point of contact (e) DUNS number (f) FSS Contract Number, if applicable (g) Type of small business, if applicable (e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and (h) a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described in paragraph 2 and 3 below. 2. The VA Greater Los Angeles Healthcare System, Healthcare for Homeless Veterans program office, is seeking a contractor(s) with sufficient facilities to provide Transitional Housing Services for approximately 50 Veterans per day using the General Harm Reduction Program (GHRP) model of care. Veterans will require case management and 24 hour monitoring. Veterans may have mental health or physical problems, may suffer from substance abuse disorder, and may have had multiple treatment failures. Through the impending contract, the VA seeks to house veterans within the GHRP model with the intention of eventually placing eligible Veterans into affordable, permanent housing. Staffing and qualification requirements include: a. Program Manager. There shall be one designated program manager who is overall responsible for contract administration and who will be available 24 hours a day, 7 days a week, 365 days a year. The program manager shall be a Licensed Clinical Social Worker (LCSW) with a current, unrestricted license in good standing with the California Board of Behavioral Health. Additionally, the LCSW shall: (1) Hold a Master's Degree in Social Work or Family Therapy (2) Have at least two years' experience providing case management services (3) Have a current Basic Life Support certification b. Case Managers. Case managers shall be provided at a staff to Veteran ratio not to exceed 1:15. Case managers shall be accessible on site during normal business hours 8am - 5pm, Monday through Friday. Case managers shall be Licensed Clinical Social Workers (LCSW) or Licensed Marriage and Family Therapists (LMFT) with a current, unrestricted license in good standing with the California Board of Behavioral Health. Additionally, the case managers shall: (1) Hold a Master's Degree in Social Work or Family Therapy (2) Have at least two years' experience providing case management services (3) Have a current Basic Life Support certification c. Paraprofessional or Direct Care (Monitors). Monitors shall be provided at a staff to Veteran ratio not to exceed 1:15. Monitors shall perform services on site, 24/7/365 and Contractor shall have one on-call monitor per day available to respond to unexpected monitoring staff shortages. Monitors shall document interactions and observations of Veterans, facilitate medication dispensing, monitor Veterans safety, and communicate with other program staff as required. Addiction counselor certification is preferred but not required. Additionally, monitors shall: (1) Hold a Bachelor's Degree in Social Work, Communication, or Sociology (2) Have at least one year of experience providing case management services (3) Have a current Basic Life Support certification 3. All other requirements shall be detailed in the impending solicitation to be issued on or about September 1, 2014. 4. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov. --End of Sources Sought Announcement--
