- Attachment 1 - Performance Work Statement Attachment 3 - Moody Instructions Attachment 2 - Pricing Schedule This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is FA4830-14-Q-0011 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-74, Defense Acquisition Circular (DAC) 91-13 (Correction) and Air Force Acquisition Circular (AFAC) 2014-0421. The North American Industry Classification System (NAICS) is 238290. The small business size standard is $14 million. This acquisition is set aside 100% for small businesses in accordance with Federal Acquisition Regulation (FAR) 19.502-2. Description: The purpose of this solicitation is to acquire a service contract to inspect, maintain and repair Vertical Transportation Equipment (VTE), to consist of all Elevators and Dock Levelers on Moody AFB, GA, in accordance with the attached Performance Work Statement (PWS). See Attachment 1- Performance Work Statement Quotations must be submitted on the attached Pricing Schedule for the following CLINs: See Attachment 2 - Pricing Schedule for Contract Line Items Note: Wage Determination does not apply. FOB Point: Destination (Moody AFB, GA.) Period of Performance: Base Year: 1 October 2014 through 30 September 2015 Option Year 1: 1 October 2015 through 30 September 2016 Option Year 2: 1 October 2016 through 30 September 2017 Option Year 3: 1 October 2017 through 30 September 2018 Option Year 4: 1 October 2018 through 30 September 2019 Provision at FAR 52.212-1, Instruction to Quoters Commercial Items applies to this acquisition. Request for Quotation (RFQ) should be submitted and shall contain the following information: RFQ NUMBER; TIME SPECIFIED FOR RECEIPT OF QUOTES; NAME, ADDRESS; TELEPHONE NUMBER OF QUOTER; TERMS OF THE EXPRESSED WARRANTY; ANY DISCOUNT TERMS FOR EARLY PAYMENTS AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS (if applicable). Contractors shall insert unit prices in the attached pricing schedule and round to the nearest cent (for example $0.02 not $0.0231) and sign below indicating prices to be used. By signing, the quoter agrees with all terms, conditions, and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. PERIOD OF ACCEPTANCE OF QUOTE: The quoter agrees to hold the prices in its offer firm through 1 October 2014 unless another time period is specified in an addendum to the solicitation. LATE QUOTES: Request for Quotations or modification of quotes received at the address specified for the receipt of quotes after the exact time specified for receipt of quotes will not be considered. EVALUATION/AWARD: IAW FAR 52.212-2, Evaluation-Commercial Items, the Government will award a purchase order to the offeror whose quotation is judged to represent the best value to the Government. Price and Past Performance, when combined, are approximately equal to Technical capability. To demonstrate Technical capability, offerors must submit a license issued by the State of Georgia which demonstrate the ability to provide services specified in this contract. Offerors must also submit key person resume(s), demonstrating state certification to repair/maintain elevators in the State of Georgia, and an Essential Services Plan, in accordance with DFARS 252.237-7023 & 7024. The price will be calculated by the summation of the base year and all option years, as well as a possible six (6) month extention. Any "teaming" arrangements utilized in the performance of this contract shall be provided to the Contracting Officer in writing at the time of award. The definition of "teaming" is as follows: The process the Contractor uses to select key suppliers and business partners; form mutually beneficial relationships; and, integrate business operations into an organizational structure designed to meet all contract requirements.
