Description

SAA Request for Quotation Form PRICING TABLE RFQ-2014-011 This announcement constitutes a Request for Quotation (RFQ). No formal, written solicitation will be issued. The attachment to this notice entitled "RFQ-2014-011 Tyvek Coveralls" describes the product requested and provides instructions on how to respond. The United States Senate Office of the Sergeant at Arms (SAA) has a requirement for Dupont White Tyvek Coverall w/zip front, elastic wrists and ankles, hood and booties, serged seams, 25ea/case, 5XL or other manufacturer equivalent to brand Dupont White Tyvek Coverall w/zip front, elastic wrists and ankles, hood and booties, serged seams. The SAA may ask that a sample of the coverall be provided by Offeror. The SAA intends to award an indefinite delivery indefinite quantity (IDIQ) type contract with fixed prices, to the responsible Offeror with acceptable past performance, and whose quotation conforms to the RFQ as provided herein and provides the best value to the SAA. The period of performance will be from October 1, 2014 through September 30, 2015. The SAA will place delivery orders under the contract which shall state the actual quantities per size. Estimated maximum quantity is 18,000 Tyvek suits. All deliveries shall be FOB Destination to the United States Senate, 6330 Columbia Park Road, Landover, MD 20785 and made within ten (10) business days of the placement of a delivery order. Questions regarding this RFQ must be submitted in writing no later than Noon EDT, July 7, 2014, to acquisitions2012@saa.senate.gov. Responses to all written questions will be posted as an amendment to this Notice. It is the responsibility of all offerors to monitor this site for the posting of the questions posed and the SAA's answers to those questions. The SAA will not respond to questions submitted after the posted deadline or to any telephone requests for information. Oral communications are not acceptable in response to this Notice. SUBMISSION DEADLINE: The completed SAA Request for Quotation Form RFQ-2014-011 must be received no later than Noon EDT, July 16, 2014. Responses shall be submitted by email to acquisitions2012@saa.senate.gov. The SAA will not accept any mail deliveries. The subject line of the email message shall be "RFQ-2014-011 Tyvek Suits." The Quotation must contain: 1) Completed RFQ Pricing Table RFQ-2014-011; 2) Three past performance references for contracts/purchase orders of similar products within the last two (2) years. Past performance references must provide: a) Name of the Customer/Company; b) Name and phone number of the primary contact with Customer/Company; and c) contract/purchase order value. EVALUATION FACTORS FOR AWARD: The following factors shall be used to evaluate offers: (1) Acceptability of item offered, in terms of whether it meets or exceeds the RFQ requirements; (2) Past performance information for the Offeror; (3) Evaluated price. Responses received without the required information will be considered non-responsive. Access by the SAA to information in any files attached to the quotation is the responsibility of the submitting party. The SAA is not responsible for any failure to access information. The Senate or the SAA will not be responsible for any costs for the preparation of responses to this announcement.
