- DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK 1.The contractor is responsible for Pathology Services and Laboratory Services for Anatomic Pathology at the VA Medical Center, 2121 North Avenue, Grand Junction, Colorado. Contractor will provide both, on site and at the contractor's site, conventional services for Anatomic Pathology and Clinical Laboratory daily, Monday through Friday. Visits should be coordinated around the times of planned frozen sections so that these can be performed on site. Anatomic Pathology services include, but are not limited to the following: grossing and reading of surgical specimens, reading of cytology slides, performing frozen sections and performing autopsies. Must be able to utilize current equipment and supplies in use at the VA Medical Center. 2.Services provided under Section C of the schedule of supplies/services will be provided at the contractor's place of business. Transportation of specimens will be provided and paid by the VA Medical Center. 3.Turnaround times will meet or exceed College of American Pathology Requirements: a.Majority of surgical specimens (900+ or more) will be reported in less than 2 days b.Cytology specimens will be reported in less than 3 days. c.Frozen Sections will be processed and initial report received at the VA Medical Center in 30 minutes or less from the time specimen is delivered to contractor's place of business d.Autopsies will be processed and completed within 30 days 4.Services will not be performed on holidays except when urgent, emergent or callback cases dictate the contractor will perform such services without further compensation except as provided herein. 5.The contractor's facility must be physically located within the local geographic area or within a 10-mile radius of the Department of Veterans Affairs Medical Center, Grand Junction, Colorado. 6.Must, by training and experience meet appointment and credentialing standards of the Medical Center Professional Standards Board for full privileging in all Pathology procedures and services required. Must be qualified and trained as demonstrated by certification by the American Board of Pathology. 7.All services performed must comply with JCAHO and CAP requirements. 8.Contractor agrees to use the VA Computerized Patient Record System (CPRS). Contractor also agrees that autopsy, surgical pathology and cytology reports will be completed in a timely fashion as defined in VA Medical Center policy and that failure to do so could result in monetary penalties (withholding payment). 9.The VA Grand Junction, Colorado contracted dictation system must be used to dictate findings and results. Hand written reports or cassette tapes are not acceptable with the exception of autopsy reports. 10.Monthly invoices from the contractor will reflect all billable services performed by procedure, by patient. 11.Contractor must adhere to documentation levels that are standard in the medical community (CMS, formerly HCFA). 12.The contractor will be available by telephone for emergencies after working hours, on weekends, and national holidays, and response time should be sufficient to ensure proper medical care for individual patients. Working hours and national holidays can be referenced in section 2.2 Special Contact Requirements, paragraph 4. 13.This contract requires the contractor's employee, who meets all of the requirements of the contract, to perform the work specified herein. A contractor's employee must be present at the VA facility and must be actually performing the required services in accordance with contract specifications or the contract cost will be reduced accordingly. 14.It is the responsibility of the contractor to have annual tuberculosis screening. 15.It is the responsibility of the contractor to report new conversions to positive PPD or to report new symptoms related to tuberculosis, if previously positive PPD, to the Grand Junction VAMC Employee Health within 10 working days. 16.It is the responsibility of the contractor to receive appropriate immunizations, including the Hepatitis B vaccine as indicated for health care workers with potential exposure to blood or body fluids. QUALIFICATIONS: Contractor shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on the contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. ?
