Description

This combined synopsis/solicitation VA261-14-Q-0747 for commercial items has been prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; a quote is being requested and a written solicitation will not be issued. A quotation in response to this notice must be received no later than 10:00am PST Friday July 11, 2014. The solicitation number is VA261-14-Q-0747 and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-75. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offerors responsibility to ensure that the quote is received. The applicable North American Industry Classification System Code (NAICS) is 339113, the size standard is 500 employees. OFFERORS MUST STATE THEIR BUSINESS SIZE when submitting their offer. This procurement is being set aside 100% small business participation. Description of Requirements: This requirement is for the name brand Zero Gravity (see attached brand name justification). CLIN 0001 Zero-Gravity Vest, Size: Medium, Quantity: 1 each CLIN 0002 Zero-Gravity Vest, Size Small, Quantity: 1 each CLIN 0003 Zero-Gravity Vest, Size Large, Quantity: 1 each CLIN 0004 Zero-Gravity Floor Unit, Includes one case of ZGD20WA-LOOP, moveable base, 77" boom arm, one 1.0mm Pb body shield, one 0.5mm Pb face shield, bayonet style body shield docking lock, enhanced sterile draping adhesion points, clip-on should shield release mechanism, anti-reflective coating on center 3 panels of head and face shield, quad rollers on column to provide a gentler column lift, dual end soft stop feature on trolley travel and boom arm locks, Quantity: 1 each FOB: Destination for delivery to South Texas Veterans Health Care System, 7400 Merton Minter, San Antonio, TX 78229 Quote shall be good for 30 calendar days after close of the request for quotes. The Government intends to issue a firm fixed price purchase order. Quotes shall be submitted electronically via email to megan.barr@va.gov.
