Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, James J. Peter VA Medical Center is currently conducting a market survey for qualified firms capable of providing "Patient Activation Survey Software License" The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 62199. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in Central Contractor Registry (CCR) (see internet site: http://www.ccr.gov). Responses to this notice must be submitted in writing (email) and received not later than June 30, 2014 1100 AM. A solicitation is not currently available. If a solicitation is issued, it will be announced on FedBizOpps at a later date, and all interested parties must respond to that solicitation announcement through FedbizOpps Online Market Place. If you have any further question, darienzo.pena@va.gov The Centers of Innovation at VA New Jersey Health Care System seeks to measure patient activation levels and provider activation levels and provide scores for each. We are also seeking to a tool to help our staff involved in health coaching and pain management develop personalized coaching plans for Veterans targeted toward specific activation levels and health conditions. Training will be needed on the use of the use of the measurement tool and coaching tool. VANJCHS Ambulatory Care seeks to purchase a license for patient activation software and associated training to: 1. Measure the activation level of 1000 Veterans during a 24 month period. 2. Incorporate findings into the Veteran's plan of care. 3. Periodically reassess the patient activation level of Veterans during the 24 month period following the purchase of the license. 4. Measure clinicians' attitudes and beliefs regarding patient self-management during a 24 month period. 5. Provide personalized health coaching, which takes into account the Veteran's activation level for up to 1000 Veterans over a period of 24 months. 6. Receive online training for up to 20 clinicians on the use of the patient activation software, the assessment tool, and the use of the coaching software. The software will score the activation levels of patients and clinicians. This score will be used to tailor health plans based upon the patient's needs and preferences. Research has shown that using customized support and tailoring coaching styles to activation levels helps patients gain confidence in managing their health which results in improved outcomes and a reduction in the number of emergency room visits and hospitalizations.
