- Sources Sought Residential Housing for ADTP/TRP Program The Jackson VA Medical Center located at 1500 Woodrow Wilson Drive, Jackson, MS 39216-5199 has a requirement for contractor to provide 24/7 housing in a homelike atmosphere with no more than two (2) veterans per room for up to 27 veterans at a time. No proposals or quotes are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS or QUOTES and NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall NOT be construed as solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The VA Medical Center is looking to award a firm-fixed contract for the period of 1 October 2014 - 30 September 2015. The NAICS Code is 624229. The expected cost range is below $14.M. All responsible sources may submit a capability statement shall be considered by the Jackson VAMC. This is not a solicitation. Successful Offeror will be required to perform with the constraints listed in the following Performance Work Statement (PWS): STATEMENT OF WORK - CONTRACT HOUSING I.Definitions a.ADTP: Addictive Disorders Treatment Program b.TRP: Trauma Recovery Program c.CARF: Commission on Accreditation of Rehabilitation Facilities d.VA: G. V. (Sonny) Montgomery Veterans Affairs Medical Center II.Contractor will provide 24/7 housing in a homelike atmosphere with no more than two (2) Veterans per room for up to 27 Veterans at a time. III.Contractor will be responsible for (A) providing and maintaining an atmosphere that is consistent with a typical American home in terms of cleanliness, furnishings and décor; and (B) contractor will ensure the environment is designed to promote an individual sense of well-being, optimism, and integration with the surrounding community (as opposed to a hospital- or dormitory-like dwelling). NOTE: In the design and decoration of the space, a priority needs to be placed on promoting a sense of hope and belief in recovery. IV.Contractor will provide adequate space and privacy in bedrooms, bathrooms and other areas. Specifically each bedroom will have adequate space to comfortably accommodate two (2) Veterans with the furniture listed on page 3 section XV. Bedrooms and bathrooms will have individual locks. The locking mechanisms will allow for the use of a staff master key as described in section 21d. V.Contractor will address Veteran or staff-initiated requests for non-urgent repairs or maintenance, including cleanliness, and provide repair so as to adequately resolve concern within seventy-two (72) hours. During instances when the contractor cannot adequately resolve the concern within seventy-two (72) hours, the contractor will generate an action plan that details mutually agreed upon actions and timelines. VI.Contractor will address emergent or large scale issues (e.g., replacing or repairing appliances) within twenty four (24) hours of being notified of the issue. If the repair cannot be completed within twenty-four (24) hours, the contractor will generate an action plan that details mutually agreed upon actions and timelines. Written records of maintenance and repairs will be kept by the contractor and available for review by VA residential staff for the duration of the contract. VII.Female Veterans a.Provisions will be made to house at least four (4) women at a time with a separate and secure sleeping and bathroom area. b.Provisions will be made so that no more than two (2) female Veterans will reside in each room. c.All female veteran bedrooms and bathrooms will have locks on their doors. d.Female bathroom doors will be locked when accessible from unit hallways or other public spaces. e.Rooms housing female Veterans must be within line of sight of the VAMC worker's station. VIII.Age & medical needs - Contractor will ensure environment demonstrates attention to the safety & security needs of Veterans in bedroom location and bathroom design (e.g., handrails; non-slip bathroom mats). Common areas a.A minimum of two (2) common areas will be provided for watching TV or movies. Additional common areas may be necessary depending on the layout of the facility. Each common area shall be large enough to comfortably accommodate at least fifteen (15) Veterans, as well as a 48-inch or larger television, and at least one (1) table to promote socialization Contractor will provide comfortable furniture, televisions (at least 48-inches of diagonal screen size) and DVD players for each common area. b.Contractor will be responsible for maintenance, repair and replacement of common area furniture and electronics. c.Contractor will be responsible for providing access to a variety of television channels comparable to basic cable or satellite programming packages. IX.Kitchen area a.A minimum of one (1) common kitchen area will be provided. b.At minimum, the kitchen area will include two (2) full size (i.e., at least 18 cubic feet) refrigerators, a microwave and basic kitchen cutlery including disposable forks, spoons and knives. X.Laundry - Contractor will provide three (3) full size washers and three (3) full size dryers for Veteran's to do their laundry. Contractor will be responsible for repair and maintenance of laundry facilities and for ensuring an adequate stock of laundry supplies (e.g., detergent, dryer sheets). XI.Recreation & Fitness a.Contractor will provide an indoor recreation and fitness area that provides access to activities such as billiards, Ping-Pong, table, card and board games, weights and cardiovascular fitness machines (e.g., treadmill). b.Contractor will provide an outside recreation area with at least a basketball hoop and court. c.Contractor will be responsible for provision, maintenance, repair and replacement of recreational and fitness equipment and games. XII.Visitation area - Contractor will provide private space, that can accommodate five people, for platonic visits with significant others and children with designated hours defined by TRP and ADTP staff. Contractor will be responsible for providing and maintaining furnishings (i.e., chairs and a table) in visitation areas. XIII.Bedroom furniture a.Contractor will provide sufficient bedroom furniture for twenty-seven (27) Veterans. Furniture will include a locker that can be locked with a user defined code but that has staff over-ride capability. It is the responsibility of the contractor to maintain bedroom furniture and to replace any that are in disrepair. It is the responsibility of the contractor to notify the VA within forty-eight (48) hours of any furniture that requires replacement. b.Mattresses - Contractor will be responsible for replacing mattresses every four (4) years or sooner, as needed. c.Pillows & bedding i.Contractor will be responsible for replacing pillows every (one) 1 year or sooner, as needed. ii.Contractor will be responsible for replacing bedding (i.e., mattress cover, pillow cases, fitted sheet, sheet, and comforter) every two (2) years or sooner, as needed. iii.Contractor will be responsible for laundering bed linens once per week or more often if requested or upon admission of a new resident. d.Towels - The contractor will maintain an adequate supply of clean bathroom towels (i.e., four (4) full length body towels and four (4) wash cloths per week) for use by the Veterans. New towels will be purchased by the contractor every one (1) year, or sooner as needed. XIV.Facility will be handicap-accessible and will provide at least two (2) bedrooms/bathrooms designed for wheelchair access. XV.Contractor will provide at least two (2) telephones per program, placed and functioning in such a way to ensure privacy for conversations. XVI.Computer Access a.Contractor will provide at least four Pentium based computers with Internet access and up-to-date word processing software6. b.Contractor will provide wireless internet access to accommodate 27 users. c.Contractor shall use up-to-date site blocking and monitoring software for all internet access to prevent inappropriate computer use. d.Contractor will provide one (1) working fax machine and two (2) printers. e.Contractor will be responsible for maintaining an adequate supply printer/fax paper, and all necessary supplies to maintain proper working order (e.g., ink). f.Contractor will be responsible for repair and replacement of computers, faxes and printers. XVII.Contractor will provide a smoking area that meets VA guidelines (see VHA DIRECTIVE 2008-052 Smoke Free Policy for VA Health Care Facilities). a.Smoking areas will not be within thirty-five (35) feet of any entrance. b.Smoking areas will be detached buildings that are accessible, heated, air conditioned and meet the Joint Commission requirements for ventilation. c.Non-smoking areas will display signage clearly indicating where smoking is not permitted (e.g., near entrances). XVIII.Facility will be either within the city limits of Jackson or within twenty (20) miles of the VA Medical Center. XIX.Supervision a.Contractor will ensure that there is qualified, in-person, staff supervision within the residence whenever Veterans are present. Staff must have constant situational awareness of all areas. Thus, if the facility has more than one wing, one floor, or more than one building, each must be staffed, including recreation and cafeteria areas when the programming schedule dictates their use. b.The VA will provide Health Technician positions to help the contractor meet the staffing requirements of the contract. Hiring, supervision, determination of work hours/leave time and all other personnel matters related to these positions will be handled by the VA. Staffing requirements, noted in point XIX.a, not met by these positions will be the responsibility of the contractor. c.Contractor will make available as needed, one or more qualified staff members to observe and ensure safety of Veterans from at least 4pm to 11pm on treatment days and from 7am to 11pm on non-treatment days. Staff observing Veterans must be trained in management of disruptive behaviors, first aid, Basic Life Support and use of emergency equipment. d.Contractor will ensure that staff must be able to quickly access locked patient bedrooms and bathrooms in the event of an emergency. e.Contractor will ensure that all employee-occupied areas support emergency calls for help, either through panic alarms, radios, emergency telephone calls, or speed-dial cell phone programming. The VA will provide emergency contact devices for VA-employed staff; Contractor will provide emergency contact devices for contractor-employed staff. f.The unit meets all requirements in the "National Fire Protection Association Life Safety Code". g.The contractor will provide an adequately-sized work station for two (2) VA staff assigned to the facility. The VA will provide all computer equipment needed by VA staff. Work station must afford secure storage of personal staff belongings and confidential documents. h.The contractor will support an alcohol and drug-free environment i.Contractor must honor therapeutic passes granted by VA treatment staff to veterans housed in the contract facility. XX.Meals a.Contractor will provide nutritionally balanced morning and evening meals on treatment days, three (3) nutritionally balanced meals a day on all other days and healthy evening snacks daily (e.g., fresh fruit, yogurt, etc ¦). b.Some residents may have special dietary needs (e.g., low fat, low sodium, or diabetic diet). Contractor will offer meals and snacks that meet the special dietary requirements of the resident Veterans. c.Meals and snacks must be compliant with current USDA Dietary Guidelines for Americans. d.Contractor will employ a registered dietician on a contract basis to review menus on at least a quarterly basis and help modify menus to meet special dietary needs as required. e.Contractor will provide copies of menus and dietician reviews to the VA on a quarterly basis. f.Unless otherwise agreed upon, breakfast will be served at 6:30am, lunch will be served at 11:30am (when Veterans present for lunch) and dinner will be served at 5:00pm. Contractor will serve meals within ten (10) minutes of agreed-upon mealtime, at least 90% of the time for each meal. XXI.Medications a.Contractor will provide secure storage of patient medications, including any controlled medications that are authorized by the Medical Director of ADTP or TRP. b.Contractor will have staffing and procedures in place to implement the VA's self-medication procedures in cases where a veteran is judged by the ADTP or TRP Medical staff to be less than fully independent in self-medication. c.Contractor will employ licensed independent medical provider able to dispense prescription medications (LPN, RN or MD). Medical provider must be available to dispense medications at intervals that coincide with Veteran's prescription, including daytime, nighttime, weekend and as-needed dosages. Medications will be dispensed within 10 minutes of prescribed dosing schedule. PRN medication will be dispensed within 30 minutes of Veteran's request. d.Contractor will provide an on-site or on-call licensed independent medical provider (RN or MD) that will be able to provide assessment, consultation and assistance for both emergency and non-life-threatening medical and psychiatric incidents. The VA will also provide a licensed on-call physician to assist with emergencies by phone. e.Contractor-employed medical staff must maintain adequate dose dispensing records and provide copies to VA staff on a weekly basis or more frequently, as needed. f.Contractor-employed medical staff will operate in compliance with Safe Medication Management program policy/procedures described in VHA MH RRTP Handbook 1162.02. g.Contractor-employed medical staff will be able to demonstrate appropriate credentials, evidence of continuing education and maintenance of licensure. STATEMENT OF WORK - CONTRACT HOUSING XXII.Transportation a.Contractor will provide transportation to and from the G. V. (Sonny) Montgomery VAMC as required for treatment purposes. b.Contractor will provide transportation to and from community-based treatment meetings and excursions (e.g., 12-step meetings, situational exposure exercises, spiritual worship, cultural or community activities, medical appointments) as consistent with Veteran's treatment plan. c.Transportation will depart within ten (10) minutes of VA scheduled departure time at least 90% of the time. d.Contractor-employed drivers will have valid driver's licenses and will obey all applicable safety and traffic laws while transporting Veterans. e.Contractor will be responsible for purchase, maintenance and timely repair of vehicles used to transport Veterans. f.Contractor will provide transportation needs dictated by inclement weather or other unanticipated concerns (e.g., pick up resident Veterans from programming early due to weather). Contractor will provide transportation within ten (10) minutes of a mutually agreed upon time. g.Contractor will meet CARF standards associated with transportation. XXIII.Treatment a.Contractor will afford residents the opportunity to attend in house and/or community based treatment meetings and excursions as consistent with the Veteran's VA treatment plan. b.Contractor will make a staff member available to attend VA let treatment team or stakeholder meetings at a frequency of no greater than once per week. Contractor will provide weekly recreational outings (e.g., bowling, baseball game, non-fast food dining) at a cost of no greater than two hundred and fifty dollars $250.00 per outing. XXIV.Admission: Clinical staff of the VA ADTP and TRP programs will determine who is eligible for housing under this contract. XXV.Discharge: Clinical staff of the VA ADTP and TRP programs will determine discharge dates. However, in consultation with ADTP and/or TRP, the contractor may discharge veterans from their housing facility (but not the treatment program) based on violation of agency's established rules. XXVI.Health and Safety a.Facility (ies) will meet all applicable health, safety, and licensing standards outlined in VHA Handbook 1162, "Mental Health Residential Rehabilitation Treatment Program", as well as all applicable CARF and Joint Commission Standards. b.Access to the facility (ies) will be restricted to a single access-controlled entrance. Closed circuit video surveillance with recording capacity will be utilized to monitor access points and public areas. The contractor will provide video surveillance and access-control equipment to meet this requirement. The contractor will be responsible for maintaining and repairing this equipment. c.All unit entry/egress doors are controlled by keyless entry locks or automatically locking/alarmed doors, or overseen by a dedicated individual. The contractor will be responsible for providing and maintaining this equipment. d.MH RRTP unit signage will be current, visible and will contain visitor, weapon, CCTV, and illicit drug and alcohol policies. The MH RRTP unit will also display current Veteran Crisis Line posters and phone number stickers. The contractor will be responsible for providing and maintaining VA approved signage. e.The contractor will be responsible for ensuring that parking lots and walkways have adequate lighting and are free of obstructions and hazards. f.The contractor must have a written plan for handling natural disasters, fires and other emergencies that is in accordance with accrediting body standards, updated annually and made available to the VA upon request. Contractor is responsible for ensuring that all staff under their employees is knowledgeable about and compliant with emergency protocols. g.Contractor will provide materials for safe containment and disposal of biohazard materials (e.g., blood, incontinence products) in a manner that maintains Veteran privacy, comfort and dignity. h.Confidentiality i.Contractor must ensure that all staff under their employment maintains Veteran privacy and confidentiality in line with Federal guidelines. ii.Contractors will alert VA staff immediately in the event that they suspect a Veteran is in imminent danger of hurting themselves or someone else. i.Contractor is responsible for ensuring that all staff under their employ treats Veterans with respect, compassion and dignity. The VA has a zero tolerance policy for any form of discrimination or sexual harassment. XXVII.Performance Requirements Summary Matrix: Performance standards define desired services. The Government performs surveillance to determine if the contractor exceeds, meets or does not meet these standards. The Government shall use these standards to determine contractor performance and shall compare contractor performance to the Acceptable Quality Level (AQL).
