(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)The solicitation number is: VA259-14-Q-0554, and it is issued as a request for quotation (RFQ). (iii)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74. (iv)This solicitation is set-aside for Small Business. NAICS: 311423 (v)Contract Line Items: a.Emergency Food Rations (vi)Description of requirements for the items to be acquired (Statement of Work): STATEMENT OF WORK (SOW) Emergency Food Rations The VA Salt Lake City Healthcare System (VASLCHCS) is in need of emergency food storage in our Food and Nutrition department. This is to comply with our disaster preparedness directive. Emergency food supplies are needed to ensure patient's nutrition requirements are meet during any potential emergency or disaster. To limit the resources and time required to prepare food in an emergency, all meals, vegetables, and fruit must be freeze dried; absolutely nothing dehydrated. The bidding vendor must meet the following requirements: "All ingredients must be freeze dried (absolutely no dehydration) "Shelf life of 25 years (15 for Gluten free) "Must feed 600 Patients at approximately 1052 Kcals daily for 7 days "Breakfast should equal 4464 servings @ approximately 1330 calories for all food options "Lunch/Dinner should equal 4416 servings @ approximately 2590 calories for all food options "Desserts/Drinks/Rice should equal 14160 servings @ approximately 380 calories for dessert options; 400 calories for Drinks; 200 for the instant white rice "Fruit should equal 4194 servings @ approximately 272 calories for all options "Vegetables should equal 5658 servings @ approximately 211 calories for all options "Gluten-Free should equal a 40 day package @ approximately 1770 calories for all options Scope The essential requirement is for the contractor to provide freeze dried products for 600 people for 96 hours. This contracting activity proposes the issuance of a FFP procurement order. (x)Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with the offer. (xi)The clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition.
