Description
Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ F1U3C44105A001; this solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74 Effective 30 May 2014. This acquisition is 100% set-aside for Total Small Business. The associated NAICS code is 532412 with a $32.5 Million size standard. This RFQ contains one (1) line items. CLIN 0001 Loader; 6 cubic yard bucket, 380 horse power, 67K lbs operating weight minimum, 2007 model or newer, rubber tires, enclosed cab with auto heat and air conditioner, auto idle system, rock guard/window protection; delivery and pick-up; maintenance (as required); (or Equal); IAW Statement of Work; 1 Each Period of Performance: 3 Months (1 July 2014 through 30 September 2014) Service shall be conducted IAW attached Statement of Work (SOW). -End Item Description- Quotes must be signed, dated and submitted via email No Later Than 27 June 2014, 01:00 pm, Alaska Standard Time (AST) to TSgt Nicholas Bastiani, E-mail: nicholas.bastiani@us.af.mil AND TSgt Gregory Hall, E-mail: gregory.hall.7@us.af.mil Facsimile Quotations will not be accepted. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (End of clause) 5352.242-9000 Contractor access to Air Force installations. CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOV 2012) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration and valid vehicle insurance certificate to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (f) Failure to comply with these requirements may result in withholding of final payment. (End of clause) EIELSON AIR FORCE BASE DENIAL OF INSTALLATION ACCESS (September 2012) (a) Eielson AFB is a "closed" installation and all personnel are subject to identity vetting prior to being granted access and/or access credentials. Individuals who may present a threat to the good order, discipline, and morale of the installation may be denied access in accordance with applicable Air Force and local instructions. (b) Circumstances under which individuals may be denied access to the installation include, but are not limited to: (1) Criminal behavior, past or present. (2) Possession of illegal substances, weapons, contraband, or other such items. (3) There is a reasonable basis to believe that an individual may pose a risk to Eielson AFB. (c) Individuals denied access to the installation will be notified that they can appeal the decision by writing a letter requesting access and stating the reason(s) why they need access to Eielson AFB. The letter should be mailed to 354 SFS/S5R, Reports and Analysis Section, 451 Division St., Suite 106, Eielson AFB, AK 99702. Upon receipt of the letter, 354 SFS/S5R will coordinate the request with applicable agencies for access determination. Once access determination has been made, a certified letter will be mailed to the requesting individual informing them of the resolution to their request. (End of clause) WIDE AREA WORKFLOW- RECEIPT AND ACCEPTANCE (WAWF-RA) In accordance with DFARS Clause 252.232-7003, Electronic Submission of Payment Request, the contractor shall submit payment requests using WAWF-RA. Information regarding WAWF-RA, including registration and customer support is available on the Internet at https://wawf.eb.mil. For correct payment processing, the contractor shall create a 2 in 1 invoice. *CONTRACTOR MUST SUBMIT 2N1 INVOICE THRU WIDE AREA WORK FLOW TO RECEIVE PAYMENT * The contractor shall use the following Department of Defense Activity Address Codes (DODAAC).
