Clauses to be added to award document Visitor Access Form - For Security Access to Site Visit Statement of Work Radon Report Specific Requirements Security Requirements Closeout Requirements Environmental Requirements Health and Safety Requirements General Requirements THIS PROJECT IS SUBJECT TO AVAILABILITY OF FUNDS The contractor shall provide all supervision, labor, tools, equipment, transportation and materials required for the installation of a radon mitigation system in buildings 103 and 120, New Boston Air Force Station, 317 Chestnut Hill Road, NH 03070. Contractor must complete the work in accordance with the Statement of Work (SOW). This project is SUBJECT TO AVAILABILITY OF FUNDS - Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. All quotes shall be available through 30 Sep 2014. This is a 100% Small Business set-aside, all responsible small business sources that submit a written quotation will be considered. GOVERNMENT FURNISHED MATERIALS: None Performance Period is within 120 days of notice to proceed. Work to be completed by: 28 Feb 2015. SUBMITTALS DUE UPON AWARD may include but are not limited to: Provide submittals a minimum of twenty-one (21) days before start of work, submit three (3) copies (electronic submittal is acceptable and preferred) of the required information to the Contracting Officer for approval, and safety plan Per Submittals Requirements Section of SOW. Lack of adequacy of references shall be at the sole discretion of the Government and may constitute grounds for disqualification. CONTRACTOR SHALL COMPLY WITH requirements of: applicable Air Force, federal, state, and local regulations. A SITE VISIT WILL BE HELD 8 July 2014 at 1300 EST. Visits to New Boston AFS will require a one week coordination for security reasons; those planning to come must RSVP by COB 1 July 2014 by submitting the attached form 5 work days in advance as per the form instructions (NOTE submit names to the PCO and Buyer for RSVP purposes but do not forward the access forms these contain Personally Identifiable Informationa and need to be sent only as indicated on the form)- No more than three individuals per firm may attend (Site visits are not mandatory, but are highly encouraged.) Location: buildings 103 and 120, New Boston Air Force Station, 317 Chestnut Hill Road, NH 03070. ESTIMATED PRICE RANGE IS LESS THAN $100,000. SOLICITATION PACKAGE WILL BE UPLOADED TO FED BIZ OPS. Any questions must be submitted no later than 11 August 2014. Evaluation will be conducted based on lowest-price technically-acceptable basis and award will be made to the lowest-priced responsible vendor with a technically-acceptable proposal methodology. Factors to be evaluated: - Has the vender submitted sufficient evidence indicating that they meet requirements of a national certification program for radon mitigation - Is the proposal in compliance with the requirements of 1.9.1.2 Contractor Experience (see attachment)
