Description

NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Request for Quotation (RFQ) is VA786-14-Q0205. No other solicitation will be issued in response to this announcement. Please submit quotes on company letterhead. This announcement will serve as both synopsis and the solicitation. Quotes in response to this combined synopsis/solicitation are due June 27, 2014 @ 2:00PM MT and shall be emailed to Derek.Reed@va.gov. Purchase order award will be made to the quoter whose quote is responsive with the lowest price to perform services as specified in this announcement. This RFQ document incorporates provisions and clauses which are in effect through Federal Acquisition Circular FAC 2005-61. The type of purchase order contemplated is firm fixed price and all quantities are estimated. The associated North American Industrial Classification System (NAICS) code for this procurement is 561730 with a small business size standard of $7.0M. This requirement is set-aside 100% for Service Disabled Veteran Owned (SDVOSB) Small Business Concerns and only qualified and eligible SDVOSB concerns may be considered for possible purchase order award. The period of performance is fourteen (14) calendar days from receipt of purchase order. The place of contract performance is the Kerrville National Cemetery, State Spur 100 (1/2 mile north of Hwy. 27), Kerrville, TX 78028. Interested parties may contact the Cemetery point-of-contact; Mr. Aubrey David at 210 820-3891 or email to schedule a site visit. The contractor shall provide prices for all line items on the attached schedule. The contractor shall provide all plant, labor, tools, parts, equipment, transportation and incidentals needed to perform work as shown on the attached schedule. Quoters must be registered in the System for Award Management (SAM) at https://www.sam.gov to be eligible to receive award. In addition, SDVOSB concerns must be have an active account in www.vetbizregitery.gov. The selected Offeror must comply with the following commercial services terms and conditions, which incorporates by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5- Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items. The following clauses are included: 52.222-50, 52.219-27, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.232-30, 52.232-34, 52.222-41, and 52.222-26. In addition the following clauses are hereby incorporated as an addendum to FAR Clause 52.212-5.
