- 1.SERVICES: The contractor will provide hemodialysis treatment for the Martinsburg Veterans Medical Center veteran inpatients as required. Contractor will make available a licensed nurse trained in extracorporeal therapy, including acute dialysis treatments, necessary to provide emergency dialysis services at the Martinsburg VAMC seven days a week, 24 hours a day, 365 days a year. The Contractor will provide appropriate contact information for emergent, immediate and routine services provided to the VAMC. The acceptable response time is within 1 hour of being contacted. The contractor shall provide all equipment in good working order (current or latest model when applicable), and meet Centers for Medicare & Medicaid Services (CMS) and Joint Commission (JC) standards for use, as well as provide treated water, Red Sense (or VA approved equivalent), equipment, supplies, and personnel necessary for performance of contract services. In addition, the contractor shall provide all repairs, upkeep, servicing, maintenance and preventative maintenance for all contractor owned equipment pursuant to the manufacturer's guidelines and applicable industry standards. Contractor shall also: "Properly clean and appropriately store all equipment as defined by the manufactures guideline associated with dialysis treatments immediately after each treatment and assist in maintaining work area. "Develop a cleaning maintenance schedule check-off and signature space for routine upkeep of equipment. "Perform preventive maintenance as necessary. 1.1.The qualified individuals providing service ("Contract Practitioner") under this contract shall perform the following duties: 1.Contractor shall furnish all personnel and equipment necessary to perform acute inpatient hemodialysis to eligible beneficiaries of the Department of Veterans Affairs Medical Center. Duties will include provide, maintain in good operating condition, and repair all dialysis and related equipment necessary for the provision of services, commercially available dialysate solutions ordered for service, tubing sets required for company provided equipment or for the provision of services, dialyzer, filters required for company provided equipment including transducer protectors and filters for portable RO equipment, fistula needle, and water quality analysis supplies. 1.1.1.All services shall be performed in accordance with standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), VAMC Martinsburg, WV policies and procedures, VA regulations, and Rules and Bylaws of the medical staff. 1.1.2.The services specified herein may be changed by written modification to this contract. The modification will be prepared by the Contracting Officer, and will not become effective until after execution by both the Contractor and the Contracting Officer. 2. QUALIFICATIONS: 2.1. Staff/Company 2.1.1 The registered nurse must be certified dialysis nurse and demonstrate independent clinical skills in the area of acute management of hemodialysis patients and demonstrates understanding of the concepts of care of patients in all stages of chronic renal disease. The RN demonstrates knowledge, skill and ability to utilize various specialized equipment to deliver treatment to ill hemodialysis patients in the acute care unit. 2.2 The company shall provide registered nurse(s) who are competent, qualified per the performance work statement and adequately trained to perform assigned duties. 2.2.1 Standards of Care: The registered nurse shall cover the range of hemodialysis services as would be provided in a state of the art health care facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by: 2.2.2 The professional standards of the Joint Commission (TJC) http://www.jointcommission.org/standards information/standards.aspx
