Description
STATEMENT OF WORK dated 6/13/2014 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 "Streamlined Procedures for Evaluation and Solicitation for Commercial Items", as supplemented with additional information included on this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation document will not be issued. The Government intends to issue a firm fixed priced contract for a base year and four (4) twelve month option periods. The solicitation number is 14-010211. This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-70 effective September 30, 2013. The associated North American Industrial Classification System (NAICS) code for this procurement is 621111 and the Small Business Size standard is $10M. This combined/synopsis solicitation is not a set aside for small business. The acquisition will be made pursuant to the authority in FAR 13.5 to use simplified procedures for commercial items. DESCRIPTION OF REQUIREMENT: The National Institutes of Health (NIH), Clinical Center, Critical Care Medicine Department (CCMD), manages a research program in the District of Columbia and at the NIH entitled: District of Columbia Partnership for AIDS Progress (DCPFAP). This program has the goal of performing research on how to improve HIV care in DC with the aim of providing a model that other urban areas can follow. The program is directed for the Critical Care Medicine Department by the Director, CCMD with the advice of a steering committee that includes the Director, Division of AIDS, NIAID and the Associate Director of the DC Department of Public Health for HIV/AIDS, Hepatitis, and Sexually Transmitted Diseases. DCPFAP needs a contractor who can provide the following support to fulfill its clinical and research missions during an initial term of one year with options to extend the term of the contract based on performance and funding: 1. A team of nurses (2), nurses manager (1), nurse practitioner (1), physicians (2) and secretary (1) who can work at the NIH Clinical Center and at DC PFAP sites in DC to recruit protocol patients and manage the care of such patients under protocols that are approved by DC PFAP leadership. 2. All health care personnel must be licensed in the District of Columbia and eligible for clinical privileges at DC PFAP sites. The work product of this contract will be evaluated based on: 1. Ability to recruit qualified personnel in time to meet all clinical and research goals. 2. CCMD evaluation of clinical competence and quality of clinical care of clinical staff. 3. Number of research protocols successfully written, approved by all regulatory agencies, fully enrolled, analyzed, presented at meetings, and published within an acceptable time frame. 4. Ability to interact effectively with DC clinics, NIH staff, regulatory bodies, and the community. Relationship with all stakeholder including but not limited to CCMD and DC PFAP leadership, community clinic management and staff, patients, community and patient groups, individual patients, NIH management and staff. The Contractor is responsible for the accuracy and completeness of the data within the SAM database, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in the SAM database after the initial registration, the Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the SAM database to ensure it is current, accurate and complete. Updating information in the SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document. FAR 52.212-2 Evaluation-Commercial Items (Addendum) Please see Statement of Work section "Evaluation Criteria" for full disclosure of selection requirements. Technical and past performance when combined is significantly more important than price, however, price remains a significant factor. The Government will award the purchase order to the responsible offeror whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical (ii) Past Performance (iii) Price Price proposed must be detailed and represent the offeror's response to the schedule of items above. The government will evaluate offers by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the option(s). Technical capability will be determined and evaluated by review of information submitted by the offeror, which must provide sufficient technical information necessary for the Government to conclusively determine that the offered services meet the technical requirements identified above. Offerors shall specifically address the specifications stated above as well as technical specifications, descriptive material, literature, brochures and other information which demonstrates the capabilities of the offeror. Past Performance will be determined and evaluated by the identification of federal, state, local Government, or private contracts for which the offeror has, in the past three (3) years, for which the offeror has provided the same service for use in same or similar applications. Provide name, telephone number, email address, contract number, past performance point of contact and dollar amount of sale. The Government is not responsible for locating or securing any information, which is not identified in the proposal, however the Government reserves the right to obtain information for use in the evaluation from any and all sources including sources outside of the Government. Offeror's shall include the firm's DUNS number with quote. 52.212-3, Offeror Representations and Certifications - Commercial Items (August 2013) - with Addenda: Representation By Corporations Regarding an Unpaid Delinquent tax Liability or a Felony Conviction Under Any Federal Law and FAR 52.204-6, Data Universal Numbering System (DUNS) Number (July 2013). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
