This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular 2005-74. The associated North American Industrial Classification System (NAICS) Code for this procurement is 811219 (Surgical and Medical Instrument Manufacturing), Size standard is $19M. This RFQ is being issued as Firm Fixed Price award. All responsible sources may submit a quotation, which will be considered by the agency. Failure to comply with the below terms and conditions may result in offer being determined as non-responsive. EVALUATION OF FACTORS: Award will be made based on Technically Acceptable, lowest price. The evaluated price will be total price to include all options. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). Technical Factors are more important than Price. Price must be determined to be fair and reasonable. A. Must be able to provide annual preventive maintenance IAW PWS para 4.1 for the Caloric Vestibular Function Stimulator. B. Must be able to provide annual calibration verification checks IAW PWS para 4.2 for the Caloric Vestibular Function Stimulator. C. Must be able to provide corrective maintenance IAW PWS para 4.3 for the Caloric Vestibular Function Stimulator. D. Must be able to provide replace parts IAW PWS para 5.1 for the Caloric Vestibular Function Stimulator. E. Must comply with operation security training IAW PWS para 7.
