Combined Synopsis/Solicitation: MEDICAL / SURGICAL SUPPLIES GENERAL: This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued. Quotes are being requested under Request for Quote (RFQ) number VA249-14-Q-0594 and no other written solicitation will be issued. This procurement is set-aside 100% for Small Business. Offerors must be registered with the System for Award Management (SAM) prior to award. Registration information can be found at www.sam.gov. Offerors must state their business size and if the item is open market or on FSS/GSA schedule when submitting offers. Responses should reflect technically acceptable new items only. Applicable shipping charges should be included on the quote. Delivery is required within 30 days ARO. No partial quotes will be considered. The Government intends to issue a firm fixed price purchase order. Offers will only be accepted electronically via e-mail to the contracting officer listed below.
